Kvinners erfaringer med psykososiale reaksjoner etter en mastektomi
Abstract
Bakgrunn
Cancer mammae er den hyppigste kreftformen både nasjonalt og internasjonalt hos kvinner. Ca. 2800 nye tilfeller av denne sykdommen oppdages årlig i Norge. Mange kvinner opplever at kreftdiagnosen gjør noe med deres identitet og verdighet, og at den derfor kan oppleves som en trussel. Sykdom kan også oppleves som en krise, hvor psykososiale behov har sitt utgangspunkt i sykdom som opplevd fenomen, altså hvordan pasienten opplever å være syk. For å kunne gi god sykepleie bør sykepleieren ha innsikt i, og en forståelse for, det sykdomsforløpet som brystkreftpasienter går igjennom på hver sin spesielle måte, og kvinnenes opplevelse av dette.
Hensikt
Hensikten med denne oppgaven er å kunne identifisere kvinners psykososiale reaksjoner etter de har gjennomgått en mastektomi, for å finne ut av ulike utfordringer og forandringer dette kan medføre i kvinnenes liv. Vi ønsker også å diskutere hvordan man som sykepleier kan møte disse kvinnene som er i en sårbar posisjon.
Metode
Det er en systematisk litteraturstudie der åtte kvalitative forskningsartikler har blitt analysert.
Resultater
Kvinnene kan erfare en rekke psykososiale reaksjoner etter en mastektomi. De mest fremtredende funnene var endringer i kroppsbildet, at overlevelse var en prioritet og hvordan de opplevde den sosiale hverdagen.
Konklusjon
Kvinnene kunne erfare psykososiale reaksjoner etter mastektomi forskjellig. Hos de fleste ble kroppsbildet negativt påvirket i form av at de kunne føle de mistet sin kvinnelighet og seksuelle identitet. Kvinnenes intime forhold og det å være i sosiale sammenhenger der det manglende brystet kunne komme til syne kunne være en utfordring. Andre hadde fokuset på å overleve og brydde seg ikke om å måtte leve uten et bryst så lenge kreften var borte. Background
Breast cancer is the most common form of cancer both nationally and internationally among women. Ca. 2800 new cases of this disease are detected anually in Norway. Many women feel like the cancer diagnosis does something to their identity and dignity and that it can be percieved as a threat. Illness can be percieved as a crisis in wich psychosocial needs are based on the disease as an experienced pneomenon, wich means how the patient experiences being sick. In order to provide good nursing care, the nurse should have insight and an understanding of the course of the illness wich breast cancer patients go thorugh in their own way, and the women's experience of this.
Objective
The purpose of this assignment is to identify women's psychosocial reactions after they've undergone a mastectomy to find out about the various challenges and changes that this can lead to in the women's lives. We also want to discuss how a nurse can meet these women who are in a vulnerable position.
Methods
It is a systematic literature study where eight qualitative research articles have been analyzed.
Results
The women can experience a variety of psychosocial reactions after a mastectomy. The most prominent findings were changes in the body image, that survival was a priority and how they experienced social life.
Conclusion
The women could experience psychosocial reactions after a mastectomy differently. At most, the body image was adversely affected in that they could feel they lost their femininity and sexual identity. The women's intimate relationship and being in social settings where the missing breast could be seen could be a challenge. Others had the focus on surviving and did not care about living without a breast as long as the cancer was gone.
