"Å stå i det" Helsepersonells erfaringer med den barnepalliative oppfølgingen i kommunen - en kvalitativ studie

Bakgrunn: I Norge er det estimert at 3500-4000 barn har behov for en barnepalliativoppfølging årlig, og fra 2017 til 2021 døde i snitt over 180 barn mellom 0-19 år årlig somfølge av sykdom. Kreft, sykdommer i nervesystemet, tilstander fra perinatalperioden ogmedfødte misdannelser er de største årsaken til dødsfallene i gruppen. Hovedmålet medbarnepalliasjon er å øke livskvaliteten for barn og familier i møte med livstruende ellerlivsbegrensende tilstander. Det er sett at helsepersonells trivsel påvirker pasienter ogpårørendes opplevelse av helsetjenesten, noe som fremmer viktigheten av å ivaretahelsepersonellet i møte med barna og familiene. Det er ingen publisert nasjonal forskningsom tar for seg helsepersonells erfaringer med den barnepalliative oppfølgingen ikommunene.
Hensikt: Belyse helsepersonells erfaringer med den barnepalliative oppfølgingen i sinkommune, egne behov i arbeidshverdagen, samt hvordan helsepersonell erfarer at barnaog familiene opplever oppfølgingen
Metode: Individuelle semistrukturerte intervjuer med helsepersonell ble gjennomført iTrøndelag i perioden juni-august 2022. Utvalget besto av sju helsepersonell medvarierende yrkesfaglig bakgrunn. Alle kommunene i Trøndelag ble kontaktet per e-postfor å kartlegge deres erfaringer med barnepalliasjon de siste fem årene. Dataene bleanalysert med Kirsti Malteruds systematiske tekstkondensering.
Resultater: Flere informanter erfarer at kommunene mangler en plan på denbarnepalliative oppfølgingen. Der organiseringen av oppfølgingen struktureres uliktmellom kommunene. Det varierer i hvilke muligheter familiene har innen praktisk hjelpog avlastning. Informantene belyste en arbeidshverdag der de opplever å ha mangearbeidsoppgaver og lite tid, hvor de trenger mer støtte fra kollegaer, ledere og debarnepalliative teamene ved sykehusene. Familiene har behov for en koordinator,kontinuitet innen tjenesten, relasjon og tillit med helsepersonell og sorgstøtte.Oppfølgingstilbud innen sorgstøtte erfares av informantene å være manglende.Kommunikasjon mellom tjenestenivåene beskrives som utfordrende, hvor epikriser oginformasjon i enkelte tilfeller uteblir. Det barnepalliative teamet ved St. Olavs hospitallykkes med samhandling og kommunikasjon med informantene i kommunen.
Konklusjon:Studien viser at helsepersonell innen den barnepalliative oppfølgingen ikommunene står i en krevende arbeidshverdag. Manglende organisering av denbarnepalliative oppfølgingen er en av årsakene til høy arbeidsbelastning både fysisk ogpsykisk, hvor de står i en mulig risiko for å bli utbrent. Det er et behov i framtiden atkommunene styrker sitt fokus på ivaretakelsen av helsepersonell. Dette ved å legge tilrette for mer kollegastøtte, økt kompetanse, og gi tid til bearbeiding av inntrykk og sorg,både før og etter barnets død. Et funn som tidligere ikke er blitt belyst er behovet for atforhåndssamtaler og beredskapsplaner inneholder klare føringer for sorgstøtte oginnlevering av hjelpemidler etter barnets død.

Background: In Norway the estimate for children needing palliative care range from3,500-4,000 annually, and from 2017 to 2021, an average of over 180 children betweenthe ages of 0-19 died as a result of disease. Cancer, diseases of the nervous system,conditions from the perinatal period and congenital malformations are listed as thebiggest causes of death. The main goal of pediatric palliation is to increase the quality oflife for children and families facing life-threatening or life-limiting conditions. It has beenestablished that healthcare professionals wellbeing influence patients' and relatives'experience of the healthcare system. There is a need to attend to healthcare personnelhealth facing children and families in need of children palliative care. To my knowledge,there is no research in Norway that describes health personnel's experiences withpalliative care for children in the municipalities.
Purpose: Describe healthcare personnel's experiences with palliative care for children intheir municipality, their own needs in everyday work, as well as how healthcareprofessionals interpret the children and families experiencing the follow-up.
Method: Individual semi-structured interviews were conducted in the county Trøndelag,during the period June-August 2022. The committee consisted of seven healthcareprofessionals with varying professional backgrounds. All the municipalities in Trøndelagwere contacted by e-mail to survey their experiences with child palliation over the pastfive years. The data was analyzed with Kirsti Malterud's systematic text condensation.
Results: Several informants experience that the municipalities lack a plan for pediatricpalliative care. Where the organization of the follow-up is structured differently betweenthe municipalities. It varies in what opportunities the families have in terms of practicalhelp and relief. The informants shed light on a working day where they experiencehaving many tasks and little time, where they need more support from colleagues,managers and the pediatric palliative care teams at the hospitals. The families need acoordinator, continuity within the service, relationship and trust with healthcarepersonnel and bereavement support. Follow-up services within bereavement support areperceived by the informants to be lacking. Communication between the service levels isdescribed as challenging, with epicrisis and information missing in some cases. Thepediatric palliative care team at St. Olav's hospital succeeds in interaction andcommunication with the informants in the municipality.
Conclusion: The study shows that health professionals within pediatric palliative care inthe municipalities have a demanding work day. Lack of organization of palliative care forchildren is one of the reasons for high workloads, both physical and mental, where theyare at a possible risk of burnout. There is a need in the future for the municipalities tostrengthen their focus on the care of health professionals. This is by facilitating morecolleague support, increased competence, and giving time to process impressions andgrief, both before and after the child's death. A finding that has not previously beenhighlighted is the need for that advanced care planning contains clear guidelines forbereavement support and the handing in of aids after the child's death.
