Sammendrag
Hensikt: Denne oppgaven tar sikte på å belyse pårørendes reaksjoner på at en person som står de nært blir utsatt for et alvorlig traume og/eller plutselig død, samt hvilke behov de har i akuttfasen og hvordan sykepleier bør handle for å imøtekomme dette. Metode: Gjennom et systematisk litteraturstudie skal temaet belyses ved hjelp av relevant litteratur, jus og aktuell forskning. Pensumlitteratur, relevante teorier og etiske prinsipper har dannet teorigrunnlaget. Videre har det blitt utført systematiske litteratursøk i databasene Cinahl, PubMED og ScienceDirect, hvorpå fire reviewartikler og fire original-artikler har blitt tatt i bruk og analysert. Resultatet har så blitt sammenfattet og diskutert opp mot teorigrunnlaget, og danner sammen grunnlaget for løsning av oppgavens problemstilling. Resultat: Sentrale funn viser at pårørende av den aktuelle pasientgruppen har høy risiko for å utvikle psykiske lidelser i form av stress, angst, depresjon og PTSD, og har et stort behov for oppfølging, da særlig gjennom adekvat informasjon om pasientens tilstand, tilgang til pasienten, og gode relasjoner med helsepersonell som for eksempel sykepleiere. Pårørendes autonomi i forhold til tilstedeværelse og deltakelse i behandlingen er også et viktig aspekt. Konklusjon: At disse behovene møtes av sykepleier kan ha stor betydning for hvorvidt sorgprosessen utvikler seg i positiv eller negativ retning hos den pårørende. Funnene i dette systematiske litteraturstudiet viser at det er viktig med systematisk og individuell kartlegging av pårørende. Det er svært viktig at man som sykepleier er innforstått med at pårørende også trenger oppfølging, og at man innehar kunnskap om omsorgsetikk og -moral, samt innehar nødvendig faglig kunnskap for å vurdere pårørendes oppfølgingsbehov. *** Background: This study's objective is to cast light on the psychological consequences of experiencing that a loved one suffers serious trauma or sudden death, which needs are most crucial in the acute phase and how the nurse should interact with the dependents to satisfy their needs. Method: These questions will be answered through a systematic review, by the use of relevant litterature, laws and available research. Pensum litterature, relevant theories and ethical principles have been used to create the theoretical basis. Furthermore, there has been conducted systematic searches in the research databases Cinahl, PubMED and ScienceDirect, which resulted in the use and analysis of four review articles and four original articles. The results were then summarized and discussed in light of the theoretical basis, which combined forms the basis for the solution to the study's primary objective. Result: Findings indicate that dependents of the said patient group faces a great risk of developing mental disorders like stress, anxiety, depression and PTSD, and has a great need of support, especially in the form of information about the patients medical status, access to the patient, and good relations to health care providers like for instance nurses. The dependents autonomy in regards of presence and participation in the treatment is also found to be an important aspect. Conclusion: The extent to which there needs are met can have a huge bearing on whether the grieving process evolves in a positive or negative direction. The findings in this systematic review suggest that systematic, thorough and individual examination of the dependents is important, and that it is crucial that nurses understand that the dependents are in need of care also. To accomplish this, nurses need to have ethical knowledge, and professional expertise to assess the dependents need for care.
