Sammendrag
Norge står i dag overfor en økende befolkningsvekst, som fører til at sykehusene må tilpasse seg for å møte fremtidens behov. Ressurser som arbeidskraft og kapasitet vil være under press, som medfører til behov for standardisering, forbedring og effektivisering av prosesser.
En av primærfunksjonene til et sykehus er sterilt gods, som blant annet benyttes i alle kirurgiske inngrep. Det stilles krav til renhold og sterilisering av instrumenter, og det er derfor viktig at sterilprosessen fungerer optimalt.
Denne bacheloroppgaven skal besvare problemstillingen om hvordan Lean tankegang kan brukes for å identifisere ulike kilder for sløsing i sterilprosessen, og videre komme med forbedrende og effektiviserende løsninger. Det har i samarbeid med St. Olavs hospital blitt undersøkt material- og informasjonsflyten mellom sterilsentralen og bevegelsessenteret, akutten og hjerte-lunge senteret, og kvinne-barn senteret.
Det er blitt gjennomført intervjuer, observasjoner og møter for innhenting av informasjon fra de ulike avdelingene, som har dannet et solid grunnlag for oppgaven. Fra denne informasjonen har det blitt brukt Lean verktøy som Value Stream Mapping for å identifisere ikke-verdiskapende aktiviteter langs verdikjeden. Både den nåværende situasjonen og den ønskede fremtidige situasjonen har blitt visualisert for alle de tre avdelingene.
Kapasiteten på uren sone på sterilsentralen er en felles flaskehals for alle avdelingene, og er avgjørende for den totale ledetiden gjennom sterilprosessen. Styringsprinsipp for bestilling er ulikt i avdelingene, og har vist seg å ha forbedringspotensial. Tatt i betraktning at oppgitt informasjon er korrekt, vil Lean verktøy som 5S og kanban bestillingsmetodikk være gode løsninger for å oppnå bedre flyt i sterilprosessen for de identifiserte problemområdene. Norway is currently facing an increasing population growth, which means that hospitals must adapt to meet the needs of the future. Resources such as manpower and capacity will be under pressure, causing a need for standardization, improvement and streamlining of processes.
One of the primary functions of a hospital is sterile goods, which are used in all surgical procedures. Requirements are set for cleaning and sterilization of instruments, therefor it is important that the sterilization process functions optimally.
This bachelor thesis will answer the question of how Lean thinking can be used to identify different sources of waste in the sterilization process, and further come up with improved and streamlined solutions.
In collaboration with St. Olavs hospital, the flow of materials and information between the Sterilization Centre and the Movement Centre, the Emergency and Cardiothoracic Centre, and the Women and Children Centre has been investigated.
Interviews, observations and meetings have been conducted to obtain information from the various departments, which has formed a solid basis for this thesis. From this information, Lean tools such as Value Stream Mapping have been used to identify non-value-adding activities along the value chain. Both the current state and the desired future state have been visualized for all three departments.
The capacity of the unclean zone at the Sterilization Centre is a common bottleneck for all departments, and is crucial for the total lead time through the sterilization process. The management principle for ordering goods is different in each department, and it has shown great potential for improvement. Considering that the information provided is correct, Lean tools such as 5S and kanban ordering methodology will provide good solutions to achieve improved flow in the sterilization process for the identified problem areas.
