Hvordan kan sykepleier ivareta psykososiale behov hos kreftpasienter som befinner seg i en beskyttende isolasjon etter stamcelletransplantasjon?
Abstract
Problemstilling: “Hvordan kan sykepleier ivareta psykososialebehov hos kreftpasienter som befinner seg i en beskyttende isolasjonetter stamcelletransplantasjon?”Bakgrunn: Ivaretakelse av isolerte pasientens psykososiale behovoppleves som utfordrende og tidkrevende. I følge statistikken er detomtrent tre tusen nordmenn som blir rammet av hematologiskekreftformer hvert år. Disse utgjør en relativ stor pasientgruppe, ogvelutviklet kunnskap om hvordan psykososiale behov kan bliivaretatt er dermed svært relevant.Hensikt: Hensikten med denne litteraturstudien, er å rette fokusetførst og fremst mot de psykososiale behovene hos isolerte pasienter,etter gjennomgått HSCT-behandling. Oppgaven omhandlerpasientens perspektiv, og retter fokuset mot helsefremmende tiltaksom forebygger psykiske og sosiale plager hos denne pasientgruppa istørst mulig grad.Metode: Denne bacheloroppgaven er en litteraturstudie, som basererseg på sju kvalitative forskningsartikler hentet fra databasene Cinahl1og Medline.Resultat: Gjennom analyseprosessen ble det avdekket trehovedtemaer: beskyttende isolasjon og psykiske plager (a),pasientens egen opplevelse av beskyttende isolasjon (b) ogsykepleierens funksjon (c), som videre er utdypet i studiensdiskusjon.Konklusjon: Litteraturstudien trekker fram at sykepleiere spiller enavgjørende rolle i ivaretakelse av pasientens psykososiale behovunder isolasjonsperioden. For å sørge for dette er sykepleierensforebyggende, helsefremmende og veiledende rolle av storbetydning. Havkis kontrollmodell anvendes som struktur fordrøftingen i denne studien. Modellen innebærer punktene kognitiv,instrumentell og emosjonell kontroll, som videre bidrar til å fremmepasientens psykososiale behov i krevende situasjoner sombeskyttende isolasjon. Fokuset rettes mot pasientens aktivt deltakse ibehandlingsforløpet, formidling av informasjon, utøve forsvarlig ogomsorgsfull sykepleie basert på velutviklet kompetanse og ikkeminst fremme opplevelse av håp og mestring. Forskningsresultaterviser også til at relasjon med pårørende og pasientens egen vilje eravgjørende for å mestre pasientrollen og den krevende situasjonen debefinner seg i. Research question: “How can a nurse preserve psychosocial needsin cancer patients who are located in protective isolation after astem cell transplantation?”Background: Preservation of psychosocial needs in isolatedpatients is perceived as demanding and time-consuming. Accordingto statistics, there are approximately three thousand Norwegianswho are afflicted with hematological cancer forms every year. Theymake up a relatively large patient group. Hence, well-developedknowledge on how to preserve psychosocial needs is exceedinglyrelevant.Aim: The purpose of this literature review is to direct focustowards the psychosocial needs of patients in isolation, afterundergoing HSCT-treatment. The thesis deals with patients´perspective and draws focus towards health-promoting measures3that prevent psychological and social ailments within thispatient-group to the greatest extent possible.Method This bachelor thesis is a literature review, which is basedon seven qualitative research articles retrieved from the databasesCinahl and Medline.Results: Through the analysis process, there were three mainthemes revealed: protective isolation and psychological ailments(a), the patient´s own experience of protective isolation (b) and thenurses’ function (c), which is further elaborated in the discussion ofthe study.Conclusion: The literature review puts forward, that nurses play acrucial role in the preservation of patients’ psychosocial needsduring the isolation period. To ensure this, the nurses preventiveand health promoting methods, as well as their guiding role is ofbig importance. Havkis control model is utilized as a structure forthe discussion in this study. The model involves the points,cognitive, instrumental and emotional control, which furthercontributes to promoting patients’ psychosocial needs indemanding situations such as protective isolation. Focus is directedtowards the patients´ active participation in the course of treatment,conveying information, exercise prudent and caring nursing basedon well developed competence, as well as promoting a feeling ofhope and mastery. Research results also point out that relation torelatives and the patients own will is crucial in order to master thepatient role and the challenging situation they are in.
