Helseindustrien opplever for tiden rask vekst, som også innebærer betydelige endringer i demografi,offentlige forventninger og teknologi. I kjølevannet av dette øker også kompleksiteten i helseindustrien,samt presset for kontinuerlig forbedring og utvikling. Som en respons til disse utfordringene,kan helsevesenet dra nytte av erfaringer fra andre industrier med hensyn til verdikjedestyring,samt ta i bruk helseinformasjonsteknologi (HIT) som anses som en avgjørende faktorfor å følge utviklingen i industrien. I tillegg har det også blitt oppdaget mangler i litteraturenrelatert til den startfasen av standardiserte pasientforløp, også kalt den preoperative fasen. Dettehar ført til en overordnet Problemstilling som studeres i detalj ved hjelp av tre sammenhengendeforskningsspørsmål:
I hvilken grad kan HIT og verdikjedestyring utnyttes for å fasilitere effektive preoperativepasientforløp?
Denne avhandlingen innledes med en introduksjon og en presentasjon av forskningsmetodikken,etterfulgt av en litteraturstudie og casestudie. Deretter blir funnene analysert og diskutert, føren konklusjon kan bli sammenfattet. Litteraturstudien innebærer å oppdage eksisterende faglitteraturom standardiserte pasientforløp, verdikjedestyring (SCM) og helseinformasjonsteknologi(HIT), som bidrar til det teoretiske grunnlaget. Casestudien i denne oppgaven bruker to forskjelligemetoder for å samle inn data, dette inkluderer to semistrukturerte intervjuer samt innsamlingav sekundære data (rapporter, Stortingsmeldinger, flytdiagrammer osv.). Data blir deretter analysertved å ta i bruk en designet verdistrømkartleggingsmodell (VSM).
I analysen ble det funnet at tverrfaglig samarbeid i samsvar med verdikjedestyring har forbedretprosesser betydelig. Derimot ble det også identifisert flere utfordringer med dagens praksis, inkludertineffektiv prioritering og vurdering, dobbeltarbeid, stor andel fysiske dokumenter, lite standardisering,lange ventetider og begrenset pasientinvolvering. Det ble også oppdaget teknologiskeutfordringer som inkluderer bruken av mange og fragmenterte HIT systemer, og mangelen på interoperabilitetog brukervennlig grensesnitt. Ved å legge vekt på standardisering og integreringpå tvers av enheter i verdikjeden, ble det konkludert at både HIT og SCM kan utnyttes i stor gradfor å legge til rette for effektive preoperative pasientforløp. I tillegg kan HIT støtte integrering vedmer effektiv lagring, deling og innhenting av informasjon. Dessuten kan HIT eliminere bruken avfysiske dokumenter og potensielt legge til rette for mer effektive preoperative prosesser ved implementeringenav kliniske beslutningsstøttesystemer (CDSS). The healthcare industry is currently experiencing rapid growth that also entails considerable changesin demography, public expectations and technology. Ultimately, this increases the complexity andpressure on healthcare organizations to continuously improve and develop. However, the healthcaresector can benefit from the lesson learned in other industries by adopting supply chain management(SCM) practices and information technology as means to keep up with the evolving industry.As a matter of fact, health information technology (HIT) is considered a critical enabler todevelop and transform the healthcare sector. However, a gap in the literature is recognized in theinitial phase of care pathways, the preoperative phase. This leads to the following overall ProblemStatement, which is explored in detail by three corresponding research questions:
To what extent can HIT and SCM practice be utilized to facilitate efficient preoperativecare pathways?
This thesis is initiated by an introduction and a presentation of the research methodology, followedby a literature study and case study. Subsequently, findings are analyzed and discussed,which ultimately leads to the conclusion. The literature study involves discovering existing academicliterature on care pathways, supply chain management (SCM) and health information technology(HIT), which provides the theoretical foundation. The case study in this thesis applies twodifferent methods as means to collect data, which includes two semi-structured interviews andcollection of secondary data (reports, white papers, flow charts, etc). Moreover, the data is analyzedby using a designed value streammapping (VSM) model.
In the analysis, it was found that multidisciplinary collaboration in accordance with SCM practicehas improved processes significantly. However, multiple challenges with the current practicewere also identified, including inefficient triage, duplication of work, multiple non-digital artifacts,lack of standardization, long waiting times and limited patient involvement. Furthermore, technologicalchallenges were also discovered, which include the use of multiple and fragmented HITsystems, and the lack of interoperability and user-friendly interfaces. Hence, it was concludedboth HIT and SCM can be significantly utilized to facilitate efficient preoperative care pathwaysby emphasizing standardization and integration across entities in the care chain. Furthermore,by utilizing HIT and its ability to store, share and retrieve information can support this integration.Besides, HIT can also eliminate the use of non-digital artifacts and potentially facilitate moreefficient preoperative processes by implementing clinical decision support systems (CDSS).
