dc.description.abstractIntroduksjon Telemetriovervaking er mykje brukt på hjartemedisinske avdelingar i dag, og har utvikla seg i over 50 år. Hjarterytma overvakast kontinuerleg, og ein er avhengig av god kvalitet på overvakinga for å unngå feildiagnostisering. Feilplassering er ein av dei vanlegaste feilkjeldene. For å redusere forstyrringar og ubalanse i signala må elektrodane vere korrekt plassert, samt at kontakta mellom hud og elektrodefeste må vere god. Hensikta med studien var å undersøke korleis sjukepleiarar etterlev rutinar og retningslinjer ved bruk av telemetriovervaking. Metode Ein kvantitativ observasjonsstudie vart utført i samarbeid med hjartemedisinske sengepostar. Det blei nytta 3 ulike observasjonsskjema, med totalt 39 observasjonar. På avdelingane er det nytta to ulike typar telemetri. Observasjonane vart utført mars 2020, rett før Covid-19 pandemien. Datasamlinga vart derfor stoppa tidlegare enn planlagt. Resultat Totalt var det 13 og 17 prosent av elektrodane feilplassert på dei to ulike telemetriane. I 83 prosent av tilfella var huda ikkje førebudd før elektrodelappane vart festa. Ved observasjon av telemetriar som allereie påkopla var 53 prosent synleg skitten. Alle telemetriane vart reingjort innan 5 minutt etter seponering. 92 prosent av pasientane som allereie hadde telemetri oppgav til sjukepleiar at dei ikkje hadde fått informasjon, Medan 58 prosent av dei som fekk kopla på under observasjon fekk informasjon om hensikta med telemetri. Konklusjon Studien har vist at det er fleire manglar ved rutinane for telemetriovervaking. Elektrodane var plassert feil og huda var ikkje førebudd. Studien avdekker også at pasientane fortel at dei ikkje har fått informasjon. Det er stort behov for klare retningslinjer og god opplæring av sjukepleiarar. dc.description.abstractIntroduction Today in coronary care units, in-hospital telemetry monitoring is often used in coronary care units today, and has developed over 50 years. The cardiac rhythm is monitored continuously, and good quality is key to avoid misdiagnoses. The misplacement of electrodes is one of the most common error in electrocardiographic monitoring. To reduce false alarms and bad signals, correct electrode placement and the quality of the electrode attachment is necessary. The aim of this study was to examine how the nurses complies with the local practice and the guidelines, when using telemetry monitoring. Method The study is a quantitative observational study, done in cooperation with coronary care units. In the observations it was 3 different forms, where we got in total 39 observations. There are two different types of telemetry used in the different coronary care units. Due to the Covid-19 pandemic in March, the observations were cut short. Results In total, 13 and 17 percent of the electrodes were misplaced. The skin was not properly prepared before attaching the electrodes in 83 percent of the observations. Observation of the telemetry that that was already in use, 53 percent were visibly dirty. However, all the telemetry was cleaned within 5 minutes after disposing. 92 percent of the patient with a telemetry, told the nurse that they had not gotten any information about telemetry monitoring. While 58 percent of the patient, that got a telemetry under observation, did get information about the telemetry monitoring. Conclusion This study has shown that there are several shortcoming in the routines for telemetry monitoring. Electrodes were misplaced and the skin was not properly prepared. It has also shown that patient felt they did not get the information they needed. This highlights the need for clearer guidelines, and better educations and instruction for the nurses at the units.
