dc.description.abstractSammendrag Innledning Sykepleiere og leger ved en sykehusavdeling i Norge diagnostiserer og beskriver behandlingen av blant annet venøse- og blandingssår til sårpasienter. De lager tilpassede prosedyrer til sine pasienter slik at sårene kan følges opp av ansatte i kommunehelsetjenesten. Sykepleierne ved sykehusavdelingen har ved enkelte hendelser observert at deres sårpasienter, som har vært gjennom utredning for sårproblematikk og fått utarbeidet en egen sårprosedyre, får mangelfull oppfølging av såret fra kommunehelsetjenesten. Avdelingen ønsker en datainnsamling for å undersøke hvordan sykepleiere ved universitetssykehuset og ansatte i hjemmetjenesten opplever kommunikasjonen og samhandlingen mellom avdelingene, da dette ikke er kartlagt fra tidligere. Problemstilling Hvordan oppleves kommunikasjon og samhandling mellom spesialist- og kommunehelsetjenesten i oppfølgingen av pasienter med venøse- og blandingssår? Metode Studien ble utført med kvalitative intervju med en semi-strukturert intervjuguide. Den kvalitative metoden som ble anvendt egner seg for beskrivelse og analysere egenskaper eller kvaliteter ved informantene (16). Utvalget av informantene besto av fire sykepleiere ved sykehuset og seks sykepleiere fra ulike hjemmetjenester i en kommune i Norge. Til å analysere datainnsamlingen ble Lindseth & Nordbergs fenomenologiske hermeneutiske metode benyttet (17). Resultat Gjennom kvalitative intervju kom det frem at det var ulike erfaringer rundt temaet i de ulike avdelingene. Ansatte i hjemmetjenesten opplevde vanskeligheter med å nå sykehusavdelingen over telefon dersom de hadde problemer med sårbehandlingen. I tillegg hadde flere ønsker til forbedring av hvordan e-meldingene ble skrevet for å sikre best mulig informasjonsflyt. Ansatte ved sykehusavdelingen opplevde også samarbeidet som distansert etter overgangen til digital kommunikasjon. Konklusjon Resultatet viser at de fleste av sykepleierne fra hjemmetjenesten var fornøyde med hjelpen de får fra sykehusavdelingen i behandling av deres pasienter med venøse- og blandingssår. Både informantene fra sykehusavdelingen og hjemmetjenesten opplever samarbeidet som distansert og ønsker et tettere samarbeid, både over digital kommunikasjon og over telefon.
dc.description.abstractAbstract Introduction Nurses and doctors at a Norwegian hospital-ward diagnose and describe the treatment of venous- and mixed ulcers among other wounds. They make personalized procedures for the wound-patient so that they can be followed up by home-care-services. The nurses with the ward have at certain events seen their patients get inadequate wound-treatment from the municipality’s home-care- services. The ward wants to survey how their own nurses and the nurses in the home-care- services experience the communication and interaction between them. Research Question How is communication and collaboration experienced between the employees of the special-health-services and the municipal health-services, in treatment of patients with venous and mixed ulcers? Method The study was conducted using qualitative interviews with a semi-structural interview guide. The qualitative method that was chosen is best suited to describe and analyze the characteristics or qualities of our informants (16). The selection of informants were four nurses at the hospital-ward and six nurses from different home-care-services. To analyze the data the method of hermeneutic phenomenological analysis of research by Lindseth & Nordberg were used (17). Findings Through qualitative interviews it was found that there were different experiences by the employees from the different departments. The employees with the home-care- services found it sometimes difficult to contact the hospital-ward by phone in need of wound-consultation. They also had suggestions for improvements with the design of the electronic-messages sent to the municipality. The wards employees also experienced the collaboration as distanced after the establishment of digital communication as the primary communication platform. Conclusion Most of our informants from the home-care-services were satisfied with the help they could get from the hospital-ward in treatment of their wound-patients. Both the informants from the municipality and the hospital-ward had a shared desire for a closer collaboration, through digital communication and by phone.
