dc.description.abstract | The most common imaging modality for spinal disorders is magnetic resonance imaging
(MRI) as it visualizes anatomical details of the spine without the need for contrast agents
or ionizing radiation. The introduction of MRI has changed the diagnosing process of
pathological spinal conditions. Degenerative, inflammatory, traumatic and tumorous
disorders can be visualized in great detail with the help of MRI. The aim of research this
project was to explore possible effects of the introduction of diagnostic MRI on surgical
spine care through the evaluation of diagnostic, predictive and epidemiological aspects.
In the first study of this thesis we found that surgery for primary intraspinal tumours in
Norway increased dramatically between 1993 and 2011 and that there seems to be a
correlation between the large increase in the availability of diagnostic MRI in Norway and
the number of surgeries for these tumours. The increase in surgery was independent of
histopathological subtype or patient age.
In the second study of this thesis we explored if radiological severity of lumbar spinal
stenosis, as measured with the Schizas classification (mild, moderate, severe, extreme) was
associated to preoperative clinical symptoms or postoperative improvement after surgery.
We found that radiological severity of lumbar spinal stenosis on MRI is not associated
with preoperative disability and pain, or clinical outcome after 1 year after surgery. The
radiological severity of stenosis has does not seem to have a clear clinical correlation and
should therefore not be overemphasized in clinical decision-making.
In the third study we examined X patients with MRI 1 day, 6 weeks and 3 months after
successful surgery for lumbar disc herniation. We found that a residual mass with
compression or dislocation of the nerve root at the operated level is common. The residual
mass usually disappears over a period of 3 months. Particularly residual masses with a high
signal on T2-weighted MRI seemed to resolve spontaneously. The radiological residual
masses seen on early postoperative MRIs may obscure interpretation in patients with
residual pain after lumbar disc surgery. | nb_NO |
dc.description.abstract | Magnetisk resonanstomografi ved kirurgisk behandling av spinale sykdommer -
Diagnostiske, epidemiologiske og prediktive aspekter:
Magnetisk resonanstomografi (MR) er den mest brukte bildeundersøkelse for å fremstille
sykdommer i ryggsøylen da den viser anatomiske detaljer i rygg og nakke uten at man
trenger kontrastmidler eller ioniserende stråling. Bruken av MR avbildning har økt mye
siden introduksjonen i Norge i 1986. Målet med forskningen som er presentert i denne
doktorgradsavhandlingen var å evaluere ulike effekter som introduksjonen av MR kan ha
hatt på kirurgisk behandling av spinale sykdommer.
I den første studien har vi analysert data fra det Norske Kreftregisteret for å beskrive
insidensraten av behandlede primære spinale svulster i Norge mellom 1993 og 2011.
Primære spinale svulster er ofte godartede og en tidligere norsk studie fra før MR epoken
fant en årlig insidensrate på 0,5 per 100.000 nordmenn. Våre analyser viser at antall
operasjoner for spinale svulster har økt betydelig i studieperioden. Økningen i kirurgisk
behandling er uavhengig av pasientalder eller histopatologisk type. I samme tidsrom har
antall MR maskiner i Norge økt fra 8 i 1993 til 125 i 2011. Økningen i kirurgisk
behandling av spinale svulster synes å sammenfalle med økt tilgang til MR-diagnostikk.
I studie to undersøkte vi sammenhengen mellom grad av lumbal spinal stenose vurdert fra
MR bilder og kliniske symptomer hos pasienter med lumbal spinal stenose . 202 pasienter
som ble operert for lumbal spinal stenose ble analysert og stenosegrad ble klassifisert på
basert på Schizas klassifikasjonen (mild, moderat, alvorlig eller ekstrem stenose). Vi fant
ingen korrelasjon mellom grad av stenose på MR og pasientens symptomer før
operasjonen eller ett år etter operasjonen.
I den tredje studien undersøkte vi pasienter som ble operert for lumbalt prolaps med MR 1
dag, 6 uker og 12 uker etter operasjonen. En restmasse i det opererte område spinalkanalen
som forsvinner i løpet av tre måneder er et vanlig MR funn også hos pasienter som er
symptomfri etter operasjonen, dette vanskeliggjør tolkingen av tidlig postoperativ MR hos
pasienter som har persisterende plager etter prolapskirurgi. | nb_NO |
