Hverdagsrehabilitering: pilotstudie om teambasert planlegging og innføring av Stjørdalsmodellen
Abstract
Rapporten omhandler planlegging og innføring av hverdagsrehabilitering
i Stjørdal kommune. Hverdagsrehabilitering (restorative
home care) ble etablert med tverrfaglig team bestående
av ergoterapeut, fysioterapeut, omsorgsarbeidere med videreutdanning
i rehabilitering og sykepleier, samt prosjektleder som
er fysioterapeut. Rehabiliteringsmodellen ble planlagt av teamet
og innført i en sone i kommunens helsetjenester. Rapporten
omhandler forskning på prosjektet. Som grunnlag for videre
utvikling og bruk av rehabiliteringsmodellen i Stjørdal kommune;
«Stjørdalsmodellen for hverdagsrehabilitering», ble det stilt tre
spørsmål om ansattes erfaringer:
A. Hvilke utfordringer og mulige nye løsninger
på pasientsentrert samarbeid erfarer teamet
i oppstartsfasen av prosjektet?
B. Hvilke utfordringer og muligheter erfarer
rehabiliteringsteamet med arbeidsmodellen etter
en innledende utprøvingsperiode på 2 måneder?
C. Hvilke erfaringer har teamet etter ytterligere halvårig
utprøving av Stjørdalsmodellen for hverdagsrehabilitering?
Et spørsmål dreide seg om suksessfaktorer og utfordringer ved
oppleggets funksjonalitet:
D. Hvilken funksjonalitet gir rehabiliteringsmodellen
internt i teamet, i samarbeid med hjemmetjenesten
og med andre aktuelle samarbeidsparter?
