En sammenligning mellom monobehandling med steroider versus kombinasjonsbehandling med hyperbar oksygen-terapi i behandling av plutselig idiopatisk sensorinevralt hørselstap

Abstract

Tittel: En sammenligning mellom monobehandling med steroider versus kombinasjonsbehandling med hyperbar oksygen-terapi i behandling av plutselig idiopatisk sensorinevralt hørselstap. Introduksjon: Plutselig idiopatisk sensorinevralt hørselstap har ukjent etiologi og flere teorier er knyttet opp mot potensiell årsak til diagnosen. I dag brukes det ulike behandlingsprotokoller hvor det vanligste tilbudet er kortikosteroider. Nyere forskning har rettet seg mer mot kombinasjonsbehandling med hyperbar oksygenterapi og de potensielle effektene pasienter kan få med denne terapiformen som tilleggsbehandling.

Problemstilling: Er kombinasjonsbehandling med kortikosteroider og hyperbar oksygen mer effektiv enn monobehandling med kun kortikosteroider i behandling av plutselig idiopatisk sensorinevralt hørselstap?

Metode: Studien er en kvantitativ litteraturstudie. Det systematiske litteratursøket er gjort i databasen PubMed. Ti studier er inkludert.

Resultat: Fire av de inkluderte artiklene omhandler kombinasjonsbehandling og de seks resterende tar for seg monobehandling. To artikler som tar for seg monobehandling viser til at intratympanisk steroidinjeksjon gir økt behandlingseffekt, både alene og i kombinasjon med orale systemiske steroider. To artikler som tar for seg kombinasjonsbehandling viser statistisk signifikant effekt med hyperbar oksygenterapi.

Konklusjon: To av fire av artiklene benyttet i vår litteraturstudie som undersøker effekten av kombinasjonsbehandling, kan tyde på at hyperbar oksygenterapi er en verdifull tilleggsbehandling til steroider. Det finnes imidlertid ikke tilstrekkelig vitenskapelig evidens som tilsier at hyperbar oksygen terapi gir signifikant økt effektivitet i behandling av plutselig idiopatisk sensorinevralt hørselstap. Det er behov for flere studier som undersøker effekten av kombinasjonsbehandling med hyperbar oksygen-terapi.

Title: A comparison between monotherapy using steroids versus combination therapy with hyperbaric oxygen in the treatment of idiopathic sudden sensorineural hearing loss.

Background: Idiopathic sudden sensorineural hearing loss has an unknown etiology, and several theories are associated with potential causes of the diagnosis. Various treatment protocols are used to treat the condition, corticosteroids being the most common. Recent research has focused more on combination therapy with hyperbaric oxygen therapy and the potential effects patients may receive with this form of addition to treatment with corticosteroids.

Research question: Is combination therapy with corticosteroids and hyperbaric oxygen more effective than monotherapy with only corticosteroids in the treatment of idiopathic sudden sensorineural hearing loss? Method: This study is a quantitative literature review. The systematic literature search has been performed in the PubMed database. Ten studies were included.

Results: This literature review consisted of four articles that included combination therapy, while the remaining six focused on monotherapy. Two articles that included monotherapy show that intratympanic steroid injection enhance treatment efficacy, both separate and in combination with oral systemic steroids. Two articles studying combination treatment shows statistical significant efficacy with hyperbaric oxygen therapy.

Conclusion: Two out of four articles used in this literature review investigates the effectiveness of combination therapy, may suggest that hyperbaric oxygen therapy is a valuable additional treatment to steroids. However, there is not enough scientific evidence to suggest that hyperbaric oxygen therapy significantly increases effectiveness in treating idiopathic sudden sensorineural hearing loss. More studies are needed to investigate the effectiveness of combination therapy with hyperbaric oxygen.
