SammendragTittel: Fear-avoidance og utvikling av langvarige korsryggplager - et systematisk litteraturstudie.
Hensikt: Hensikten med denne oppgaven er å få økt kunnskap om assosiasjonen mellom fear-avoidance og utviklingen av langvarige plager hos pasienter med akutte og subakutte ryggplager uten røde flagg.
Metode: Kvantitativ litteraturstudie, der et systematisk litteratursøk ble gjennomført i Medline, EMBASE, PsychInfo & AMED. Inklusjons- og eksklusjonskriterier ble brukt for å filtrere alle artiklene. Kvalitetsvurdering av studiene ble utført ved bruk av QUIPS-verktøy.
Resultat: 4 prospektive og 1 retrospektiv kohortstudie ble inkludert. Fear-avoidance var signifikant assosiert med sykefravær og arbeidsstatus i 2 av 2 studier som så på disse utfallsvariabelene. Fear-avoidance var signifikant assosiert med funksjonsnedsettelse og smerteintensitet i 1 av 3 studier som så på disse utfallsvariablene. 2 av 3 fant ingen assosiasjon mellom fear-avoidance og smerteintensitet og funksjonsnedsettelse.
Konklusjon: 3 av 5 studier fant signifikant assosiasjon med fear-avoidance og deres respektive utfallsparametere. Studienes metodikk varierte betydelig med hensyn til deres utfallsvariabler, konfundere som ble justert for, definisjon av smertevarighetsfaser og setting. I tillegg kan det komplekse samspillet mellom individuelle og miljømessige faktorer på adferd, smerte, funksjon, sykefravær og arbeidsstatus variere. Disse variasjonene gjør det utfordrende å forutsi assosiasjonen mellom fear-avoidance og utviklingen av langvarige korsryggplager. AbstractTitle: Fear-avoidance and the development of chronic low back pain - a systematic review.
Objective: Investigate whether fear-avoidance is associated with the development of chronic low back pain in patients with acute and subacute low back pain without red flags.
Methods: A systematic search of MedLine, EMBASE, PsychInfo and AMED was carried out. Inclusion and exclusion criteria were used to filter all articles found. Selected articles were reviewed using the QUIPS-tool.
Results: 4 prospective and 1 retrospective cohort studies were included. Fear-avoidance was found to be significantly associated with sickness absence and work status in the two studies using these outcome parameters. Fear-avoidance was found to be significantly associated with disability and pain intensity in 1 of 3 studies using these outcome parameters. 2 of 3 found no association between fear-avoidance and disability and pain intensity.
Conclusion: 3 of 5 studies found fear-avoidance to be significantly associated with their respective outcome parameters. The studies` methodology varied considerably with regard to outcome parameters, confounders adjusted for, definitions of phases of pain duration (i.e. episode duration) and the setting. Additionally, individual and environmental factors` impact on behavior, pain, function and sickness absence/work status vary. These variations make it challenging to conclude whether fear-avoidance is associated with the development of chronic low back pain in patients with acute and subactute low back pain without red flags. Further research is needed to determine fear-avoidance`s association with chronic development in this population, relative to other individual and environmental factors.
