Sykepleieres ivaretakelse av munnhygiene hos pasienter med hoftebrudd på sykehus

Abstract

Introduksjon: Det typiske for pasienter med hoftebrudd er høy alder og nedsatt funksjonsnivå. Mange av disse pasientene har derfor økt behov for omsorg under innleggelsen på sykehus, herunder hjelp til å ivareta munnhygiene. Dårlig munnhygiene øker risikoen for livstruende sykdommer og infeksjoner. Sykepleieres ivaretakelse av munnhygiene hos pasienter er derfor en vesentlig del av omsorgen som gis. Til tross for dette viser tidligere fag- og forskningslitteratur at munnhygiene er en nedprioritert og forsømt sykepleieoppgave. På bakgrunn av dette undersøker denne studien hvilke faktorer som påvirker sykepleieres ivaretakelse av munnhygiene hos pasienter med hoftebrudd på sykehus.

Metode: Dette er en kvalitativ studie. Det ble gjennomført intervjuer av fire sykepleiere i mars 2022. Dataene fra intervjuene ble analysert på en fremgangsmåte kalt meningsfortetning.

Resultat: Resultatene fra studien viser at sykepleierne ga uttrykk for at de opplevde at de ikke hadde tilstrekkelig fokus på munnhygiene, men at de anså området som en del av sitt ansvar som sykepleier. Resultatene viser også at sykepleierne oppga at de opplevde at mangel på tid og motstand fra pasientene var faktorer som førte til at munnhygiene ble nedprioritert og forsømt. Sykepleiernes rutiner og kunnskap og kompetanse om munnhygiene kan ses på som faktorer som bidro til at munnhygiene ble nedprioritert og forsømt.

Konklusjon: Det kan tenkes at munnhygiene er en sykepleieoppgave som nedprioriteres og forsømmes når det er lav bemanning og at sykepleiere har behov for kontinuerlig kunnskap om og opplæring i munnhygiene. Studien belyser sykepleieres meninger og opplevelser knyttet til ivaretakelse av munnhygiene hos pasienter på sykehus.

Introduction: The typical thing for patients with hip fractures is old age and reduced level of function. Many of these patients therefore have an increased need for care during hospitalization, including help to maintain oral hygiene. Poor oral hygiene increases the risk of life-threatening diseases and infections. Nurses´ maintenance of oral hygiene in patients is therefore an essential part of the care provided. Despite this, previous professional and research literature shows that oral hygiene is a downgraded and neglected nursing task. Based on this, this study examines the factors that affect nurses´ maintenance of oral hygiene in patients with hip fractures in hospital.

Method: This is a qualitative study. Interviews were conducted with four nurses in March 2022. The data from the interviews were analyzed in a procedure called densification of meaning.

Result: The results from the study show that the nurses expressed that they felt that they did not have a sufficient focus on oral hygiene, but that they considered the area as part of their responsibility as a nurse. The results also show that the nurses stated that they experienced that lack of time and resistance from the patients were factors that led to oral hygiene being downgraded and neglected. The nurses´ routines and knowledge and competence about oral hygiene can be seen as factors that contributed to oral hygiene being downgraded and neglected.

Conclusion: It is conceivable that oral hygiene is a nursing task that is downgraded and neglected when staffing is low and that nurses need continuous knowledge of and training in oral hygiene. The study sheds light on nurses´ opinions and experiences related to maintaining oral hygiene in patients in hospitals.
