Bakgrunn: SARS-CoV-2-viruset ble først rapportert i Kina i desember 2019. Viruset spredte seg raskt over hele verden og utbruddet ble erklært som en pandemi 11. mars 2020. I løpet av to år ble det registrert nesten 1.5 millioner Covid-19 smittetilfeller, og ca 2500 dødsfall i Norge. Ved alvorlig sykdomsforløp av Covid-19 kan pasienter bli henvist til CT-thorax, for å kartlegge alvorlighetsgrad. I Norge blir det brukt standard dose protokoll ved CT-thorax av Covid-19 pasienter.
Hensikt: Hensikten med oppgaven er å sammenligne som CT-thorax Lavdose- og Standarddose protokoller av Covid-19 pasienter, og deres stråledose og bildekvalitet ved bruk av forskning og annen faglitteratur.
Metode: Denne litteraturstudien er basert på sju forskningsartikler. Dette består av fire randomiserte kontrollerte studier, en metaanalyse, en retrospektiv datainnsamlingsstudie av en surveyundersøkelse og en retrospektiv datainnsamlingsstudie av observasjoner.
Resultat: Ved bytte fra Standard-dose protokoll til Lavdose protokollen ble det mellom 50-92% reduksjon i stråledose. CT-bildekvaliteten ble derimot dårligere ved Lavdose protokollen.
Konklusjon: Strålereduksjon blir oppnådd ved å minke mAs (20-50), bruke høy pitch (>1), tykke snitt, bruke aktiv AEC og iterativ rekonstruksjon. Nyere CT-maskiner (nyere enn 2016 installasjonsår) ble også anbefalt. God bildekvalitet var avhengig av iterative rekonstruksjonsmetoder, CT-maskinalderen (nyere enn 2016) og det var spesielt avhengig av høy nok mAs til å unngå for lav SNR. 5 av 6 forskningsartikler anbefalte lavdoseprotokollen framfor standard dose protokollen. Lavdoseprotokollen viste stor potensial for strålereduksjon og god nok bildekvalitet for gransking. Background: The SARS-CoV-2-virus was first reported in China in December of 2019. The virus quickly spread all over the world and the outbreak was declared a pandemic on the 11th of March in 2020. Norway had registered ca 1.5 million cases of Covid-19 infection, and ca 2500 deaths in two years. Covid-19 patients with severe symptoms could get referred to a CT-thorax, in order to establish the severity of the illness. In Norway a CT-thorax with a Standard-dose protocol was used when scanning Covid-19 patients.
Purpose: The purpose of this study was to compare CT-thorax Low-dose and Stand-dose protocols of Covid-19 patients, and the radiation dose and image quality of the protocols by using scientific research and other literature.
Methods: This literature study was based on seven scientific articles, of which four include randomized controlled studies, one meta-analysis, one retrospective data collection analysis of a survey, and one retrospective data collection analysis of observations.
Result: When switching from the Standard-dose protocol to the Low-dose protocol, there was a 50-92% reduction in radiation dose. CT image quality, on the other hand, deteriorated with the Low Dose protocol.
Conclusion: Radiation reduction is achieved by reducing mAs (20-50), using high pitch (> 1), thick slices, using active AEC, and iterative reconstruction. Newer CT machines (newer than installation in 2016) were also recommended. Good image quality was dependent on iterative reconstruction methods, the CT machine age (newer than 2016) and was particularly dependent on high enough mAs to avoid too low SNR. 5 out of 6 research articles recommended the low-dose protocol over the standard dose protocol. The low-dose protocol showed great potential for radiation reduction and gives good enough image quality for diagnostic use.
