dc.description.abstractÅrlig får rundt 3.1% av pasienter ved norske sykehus infeksjoner som oppstår i forbindelse med operasjoner (SSI) (St.Olavs, 2019). Flere faktorer vil forebygge infeksjoner, men det er essensielt å holde pasienten i normotermi (over 35.0 ℃). Basert på målinger, vil en pasient tape rundt 82 W/m2 ved en kjernetemperatur på 35.0 ℃, med omrøringsventilasjon rundt 23.0 ℃. En pasient vil generere rundt 40W/m2, i tillegg til at kirurger og lys vil gi rundt 9 W/m2, noe som indikerer at en bør tilføre rundt 33 W/m2 for å unngå hypotermi som oppstår under 35.0 ℃. I korte operasjoner er det ikke sikkert at kroppstemperatur vil rekke å falle under 35.0 ℃, men en ekstern varmekilde bør brukes i lengre operasjoner, samtidig som det benyttes et godt isolasjonsmateriale på pasient. Basert på målinger vil en varmemadrass på 37 ℃ generere rundt 31 W/m2 og infrarød varmelampe på 2000W kan bidra med rundt 38 W/m2. Entalpi er en funksjon av lufttemperatur og fuktighetsnivå. Operasjonsrommet som er undersøkt ved St. Olavs har konstant lufttemperatur omkring 23.0℃, så det er kun uteluftens fuktighet som påvirker entalpien i rommet. Temperaturen på luften påvirker konveksjonstapet, men fuktigheten var lite varierende under operasjoner, ingen god korrelasjon ble funnet mellom kjernetemperatur hos pasienter og entalpien til rommet. Konklusjonen er at fuktighet vil ha liten påvirkning på varmetapet til pasient, noe teoretisk modell for varmetap også bekrefter. Fuktighet er allikevel viktig under kirurgi, men på en annen måte; For lav kan gi tørr hud og medføre at sår gror tregere. For høy kan gi gode forhold for mikroorganismer og redusere svetteopptak fra pasient. Standarder og anbefalinger har forskjellige grenseverdier for inneklima, men norske anbefalinger sier at luftens relative fuktighet bør ligge mellom 40-60% i et operasjonsrom (Solem Aune, 2014). Dette er kun mulig å opprettholde i de varmeste sommermånedene, uten luftbefuktning. Luftfuktigheten i Trondheim ligger generelt under 20% relativ luftfuktighet i alle andre måneder, ved lufttemperatur 23.0 ℃. En dampbefukter ble simulert i simuleringsprogrammet IDA ICE og viser at operasjonsrommet årlig bruker rundt 1348 kWh/m2, uten befukter. Det høye tallet skyldes at operasjonsrommet krever over 20 luftskifter i timen. Ved nedre grenser for relativ luftfuktighet på 30,40 og 50%, vil energikonsumet for ventilasjon stige med 42, 193 og 257%. Dette viser hvor energiintensivt befuktning av operasjonsluft er, og det må reguleres strengt. Veiledning til teknisk forskrift (TEK17) må bli fulgt i alle norske bygge- og rehabiliteringsprosjekt. Den setter årlig energikrav på maksimalt 225-265 kWh/m2 for sykehus. Ved kontinuerlig bruk av befukter i flere operasjonsrom, vil dette være vanskelig å oppfylle.
dc.description.abstractAnnually around 3.1 % (St. Olavs, 2019) of the patients in Norwegian Hospitals get surgical site infections. Several factors will help prevent infections, but it is important to keep the patient in normothermia (above 35.0 ℃). Based on measurements, a patient will lose around 82 W / m2 at a core temperature of 35.0 ℃, with use of mixing ventilation at 23 ℃. A patient will also generate about 40W / m2 in internal heat. In short operations, it is not certain that the temperature will fall below that limit, but an external heat source is needed to keep the patient above 35.0 ℃ in longer operations. Based on experiments, a heating mattress of 37 ℃ will generate around 31 W / m2, while an IR lamp of 2000W around 38 W / m2. Staff and lightning will generate around 9 W/m2. Enthalpy is a function of air temperature and humidity level. The operating room that was examined at St. Olav's was constantly at 23.0 ℃ air temperature, so it will only be the humidity of the outside air that affects the enthalpy. The temperature of the air affects the convection heat loss but do not vary in the surgeries. The humidity had low variations during operation, and no good correlation was found between the core temperature of the patient and the enthalpy of the room air. It can be stated that the humidity of the air will have low contribution on the heat loss for the patient. The theoretical heat loss model also confirms this. Humidity is still important during surgery but in another way; Too low can cause dry skin, and that wounds heal more slowly. Too high can provide good conditions for microorganisms and reduce the air's ability to absorb sweat. The standards and recommendations all state different limits, but the Norwegian recommendation states that the relative humidity of the air should be between 40-60% in an operating room (Solem Aune,2014). This is only possible during summer, without humidification, as humidity in Trondheim is generally below 20% relative humidity at an air temperature of 23.0 ℃. The simulation program IDA ICE showed that the ventilation energy consumption for an operating room was around 1348 kWh / m2 during a year, without humidifier. This is a large number due to over 20 air changes per hour. At the lower limit of 30, 40 and 50% RH, respectively, energy consumption will increase by 42, 193 and 257%. This shows how energy-intensive it is to humidify operating rooms in Norway, and one must control it strictly. Guidance on technical requirements for buildings must be followed in all construction projects in Norway, and the latest version sets requirements for 225-265 kWh / m2 for hospitals. With continuous humidification in several operating rooms, this will be difficult to maintain.
