SAMMENDRAGBakgrunn: I 2015 introduserte Livmorhalsprogrammet i Norge et pilotprosjekt med primær HPV-basert screeningtest for halvparten av kvinner i alderen 34-69 år i tre fylker, inkludert Trøndelag. Konsekvensene av denne omorganiseringen har ikke blitt evaluert.
Mål: Å evaluere omorganiseringen av screeningprogrammet fra cytologi til HPV-basert screening. Er indikasjonene og resultatene av behandlingen de samme?
Metoder: En retrospektiv kohortstudie som inkluderer alle kvinner som gjennomgikk cervikal konisering ved St.Olav’s hospital i Norge i 2014 og 2015. Kliniske data og data angående behandling ble analysert.
Resultater: Totalt gjennomgikk 667 kvinner konisering i studieperioden. Flere kvinner ble konisert i 2015 enn i 2014, henholdsvis 419 mot 248. Flere kvinner tok celleprøve på bakgrunn av deltakelse i screening, oppfølging av tidligere lavgradige lesjoner eller HPV-positive tester i 2015 enn i 2014 (p=0,034). Vi fant ingen forskjell i den histologiske diagnosen av livmorhalsbiopsiene før konisering eller i konisatet.
Konklusjon: Etter omorganiseringen av screeningen hadde flere kvinner deltakelse i screening og oppfølging av lavgradige lesjoner/HPV-positiv test som indikasjon for celleprøve. Dette ble ikke reflektert i den histologiske diagnosen, der de fleste kvinnene hadde høygradige cervikale prekankrøse lesjoner. ABSTRACT Background: In 2015, The Norwegian Cervical Cancer Screening Programme introduced a pilot project with primary human papillomavirus HPV based screening for half of the women in age group 34-69 years in three counties, including Trondelag. The consequences of this reorganization have not been evaluated.
Objective: To evaluate the reorganization of the screening program from cytology to HPV-based screening. Are the indications and results of treatment the same?
Methods: A retrospective cohort study, including all women undergoing cervical conization at St.Olav’s Hospital in Norway in 2014 and 2015. Clinical data and data regarding treatment were analyzed.
Results: A total of 667 women underwent conization in the study period, more women in 2015 than in 2014, 419 vs. 248, respectively. More women had Pap-smears taken due to screening participation or follow-up of previous low-grade lesions or HPV positive tests in 2015 than in 2014 (p=0.034). We found no difference in the histological diagnosis of the cervical biopsies prior to conization or in the cone specimen.
Conclusion: More women had screening participation and follow-up of low-grade lesions/HPV positive test as indication for Pap-smears taken after the screening reorganization. This was not reflected in the histological diagnosis, where most women had high-grade cervical pre-cancer lesions.
