dc.description.abstractTemaet for avhandlinga ligg i skjeringsfeltet mellom psykisk helse og barnevern, og dreiar seg om psykiske vanskar hos norske barn plasserte utanfor heimen av barnevernet. Det overordna siktemålet er å bidra til auka kunnskap om plasserte barns psykiske helse; herunder å utforska førekomst, variasjon, og utvikling av psykiske vanskar. Som teoretisk utgangspunkt for avhandlinga er valt utviklingspsykopatologi og resiliensteori, som begge byggjer på ei transaksjonell forståing av menneskeleg utvikling og fungering. Gjennom tre delstudiar siktar avhandlinga mot å undersøkja tydinga av risiko- og beskyttande faktorar på ulike nivå; som kjenneteikn ved barnet, familie/nettverk, sosioøkonomiske forhold, omfang av stressande livshendingar og kontakten med barnevernet. Dei tre delstudiane kan sjåast både som sjølvstendige forskingsarbeid og som arbeid som byggjer på kvarandre. Dei to første delstudiane dreiar seg om førekomst og variasjon i barna sine psykiske vanskar kort tid etter at dei som 6-12 åringar blei plasserte utanfor heimen av barnevernet (n=109), medan den siste delstudien gjeld stabilitet og endring i barna sine psykiske vanskar frå plasseringstidspunktet og 7-8 år fram i tid (n=70), då barna var i alderen 13-20 år. Barna sine psykiske vanskar og prososiale åtferd har blitt kartlagt ved hjelp av eit standardisert kartleggingsinstrument (The Revised Rutter Scales). Informantar i delstudiane har vore barna sine saksbehandlarar i barneverntenesta, barna sine foreldre/omsorgspersonar og barna sine lærarar. Hovudfunn frå den første delstudien (artikkel 1) var at minst 70 % av dei plasserte barna hadde symptom på psykiske vanskar, og gutar oftare enn jenter. Dersom plasseringa i hovudsak skjedde pga. problem knytt til foreldra, hadde barna lågare psykiske vanskar enn dersom plasseringa skuldast samhandlingsproblem mellom foreldre og barn. Desse funna vart følgde opp og utforska vidare i den andre delstudien (artikkel 2), der det blei stilt spørsmål ved om, og i tilfelle kvifor, barn som blei plasserte utanfor heimen pga. foreldres rusproblem hadde lågare psykiske vanskar enn barn plasserte av andre grunnar. Fleire gruppeskilnader vart funne mellom barn plasserte pga. foreldres rusproblem og barn plasserte av andre årsaker. Den viktigaste forklaringsvariabelen for skilnaden i psykiske vanskar var barna si prososiale åtferd, som var langt meir framståande hos barn plasserte pga. foreldres rusproblem enn hos dei andre barna. Det å bli plassert pga. eigne åtferdsvanskar var også ein viktig forklaringsvariabel, medan variablar som kjønn, disiplinproblem eller sosioøkonomiske forhold ikkje hadde nokon effekt. Ein konklusjon i studien var at prososial åtferd kan sjåast som ein viktig beskyttande faktor mot psykiske vanskar, noko som er eit argument for sterkare satsing på tiltak som fremjar prososial utvikling. I den siste delstudien (artikkel 3) vart funn frå dei to første studiane følgde opp, med føremål å studera stabilitet og endring i barna sine psykiske vanskar frå plasseringstidspunktet og 7-8 år framover i tid. Eitt hovudfunn for barna som gruppe, var ifølgje lærarane sine rapportar at dei hadde langt lågare psykiske vanskar på oppfølgingstidspunktet enn på plasseringstidspunktet, noko som kunne tyda på at barna hadde hatt «nytte» av plasseringa utanfor heimen. Ifølgje omsorgspersonane sine rapportar derimot, var det inga endring i omfanget av psykiske vanskar. Vidare analysar basert på individuelle endringsskårar, viste derimot at mellom halvparten og fire femdelar av barna faktisk hadde lågare psykiske vanskar på oppfølgingstidspunktet. Fordi mange av endringane ikkje var store nok til at barna kryssa den kliniske grensa mellom høgskårarar og lågskårarar, vart ikkje den positive utviklinga fanga opp av gruppeanalysane. Barn med høgare initiale vanskar hadde ei meir positiv utvikling ifølgje begge informantgrupper, medan barn med lågare initiale vanskar hadde ei meir negativ utvikling, særleg ifølgje omsorgspersonane. Fleire forhold knytt til barna sin bakgrunn og forhold ved plasseringa (jfr. artikkel 1 og 2) hadde samanheng med endringane i barna sine psykiske vanskar over tid, men alle desse forholda mista effekten og vart forklarte av barna sine vanskar på plasseringstidspunktet. Ein hovudkonklusjon frå den tredje delstudien, var at barna si utvikling var prega av individuell variasjon meir enn av felles utviklingstrekk. Å behandla plasserte barn som ei homogen gruppe vil derfor kunna vera misvisande, fordi ein står i fare for å oversjå individuelle endringar som er viktige for det enkelte barn og dei vaksne kring barnet. I artiklane er drøftingstema nært knytt til dei empiriske funna, og samanlikna med anna forsking. I «kappa» er utvalde hovudfunn drøfta på eit meir overordna teoretisk nivå og relatert til sentrale barnevernfaglege tema. Dei overordna drøftingstema er: korleis kan vi forstå den mangedobla førekomsten av psykiske vanskar hos plasserte barn; tydinga av direkte versus indirekte risikofaktorar for utvikling av psykiske vanskar; korleis har offentlege omsorgsarrangement ivareteke barnas psykiske helse; og tydinga av kunnskap om individuelle variasjonar i utvikling. I avsluttingskapitlet blir det reflektert omkring implikasjonar av funna for forsking og praksis i lys av gjennomgangstemaet for avhandlinga, som ligg i skjeringsfeltet mellom psykisk helse og barnevern.nb_NO
dc.description.abstractThe theme of the thesis is situated in the intersection between mental health and child welfare, focusing on mental health problems among Norwegian children in out-of-home care. The overall aim of the thesis is to contribute to the knowledge base of placed children’s mental health conditions, by exploring the prevalence, the variation and the development of mental health problems. The theoretical basis of the thesis is developmental psychopathology and resilience theory, which are both founded in a transactional understanding of human development and functioning. Through three studies/articles the thesis has examined the impact of risk and protective factors on different levels, like characteristics of the children and the families, social networks, socioeconomic conditions, stressful life events and contact with the child welfare services. The three studies are designed both as independent research studies covering specific issues, and as studies building on each other in a successive way. The two first studies examined the prevalence and variation in the children’s mental health problems shortly after the time of out-of-home placement when the children were aged 6-12 years (n=109), while the third study examined stability and change in the children’s mental health problems from the time of placement and 7-8 years onwards when the children were aged 13-20 years (n=70). The children’s mental health problems and prosocial behavior were assessed by a standardized instrument (The Revised Rutter Scales). The information in the studies was reported by the children’s child welfare workers, their parents/caregivers and their teachers. A main finding from the first study (paper 1) was that 70 % or more of the placed children had symptoms of mental health problems, the boys more often than the girls. Furthermore the study reported that children placed out-of-home mainly due to parental problems had lower mental health problems than children placed due to interactional problems or child neglect and abuse. The findings from the first study were followed up in the second study (paper 2), focusing on if and why children placed because of parental substance abuse had lower mental health problems than children placed for other reasons. Besides documenting several group differences between children placed because of parental substance abuse and children placed for other reasons, the study found that the most important variable explaining the group difference in all subtypes of mental health problems was the level of prosocial behaviour in the children. Being placed for behavioural problems also explained a significant part of total difficulties and conduct problems, while variables like gender, discipline problems and socioeconomic conditions did not have a significant effect. The conclusion of the study was that prosocial behaviour might be regarded as an important protective factor against mental health problems, which is an argument for increased efforts in programs aimed at enhancing prosocial development. The third study (paper 3) followed up important findings from the two first studies, aiming at exploring stability and change in the children’s mental health problems from the time of placement to the follow-up time 7-8 years later. One main finding was that according to the teachers, the children as a group had far lower mental health problems at the follow-up time, indicating that the children might have benefited from the out-of-home placement. According to the caregivers however, there was no change in the level of mental health problems. Further analyses based on individual change scores, showed that between one half and four fifths of the children actually had lower difficulty scores at the follow-up time according to both informant groups. But in spite of a positive development, many of the children did not cross the cut-off line between high scorers and low scorers, for which their positive development did not show in the group analyses. Interestingly, children with higher initial problems had a positive development according to both informant groups, while children with lower initial problems had a more negative development according to the caregivers. Several pre-placement and placement variables were associated with the change in the children’s mental health problems from the time of placement to the follow-up time according to both informants. However, all the predictors were accounted for by the strong effect of the children’s problem scores when entering care. The findings from the third study indicate that there was a large variability in the children’s development over time. Thus treating children in out-of-home care as one homogeneous group, which has often been done in previous research, can give the false impression that placed children are more or less following the same developmental path over time. The discussions in the three papers are closely related to the empirical findings and compared with previous research findings. In the overall discussion in the thesis, a selection of the main findings is discussed on a more theoretical level and related to central child welfare issues. The central issues are: How can we understand the high prevalence of mental health problems in placed children; the impact of direct versus indirect risk factors on children’s mental health problems; how are the placed children’s mental health conditions taken care of by public out-of-home arrangements; and the importance of knowledge about individual differences in development. The final chapter reflects upon implications of the findings for practice and research, in the intersection between mental health and child welfare.nb_NO
