Abstract

Tittel: Pasientomsorg i helsesystemers rammer

Hensikt: Denne oppgaven vil se nærmere på hvordan ytre rammebetingelser påvirker nettopp sykepleieres arbeid i hjemmesykepleien, og derav pasientsikkerhet og kvalitet på omsorg.

Problemstilling: Hvordan påvirkes helhetlig sykepleie og pasientsentrert omsorg i hjemmesykepleien av ytre rammebetingelser som samhandlingsformen og styringsprinsipper inspirert av New public management?

Metode: Oppgaven er en litteraturstudie baser på systematisk tilnærming av hovedsakelig kvalitative forskningsartikler, sykepleieteorien til Kari Martinsen, samt faglitteratur, yrkesetiske retningslinjer, lover, forskrifter og egne erfaringer.

Resultat: Studiene viser til flere utfordringer som tidspress, underbemanning og økt fokus på resultat, som bidrar til at helhetlig sykepleie og pasientsentrert omsorg er vanskelig å utføre i stor nok grad.

Konklusjon: Hjemmesykepleiens organisering, kan true helhetlig sykepleie og pasientomsorgen. Økonomiske styrings prinsipper blir i økende grad prioritert. Dermed kan det stilles spørsmål om nåværende organisering har riktig fokus på pasienten. Tatt studiene i betraktning stiller jeg videre spørsmål om primærhelsetjenesten har de nødvendige ressursene og om nåværende ressursene blir prioritert riktig for utførelse av helhetlig sykepleie og god pasientomsorg.

Title: Patient care in the framework of health systems

Purpose: This task will look into how external framework conditions affect nurses' work in home care, in the light of patient safety and quality of care.

Issue: How is holistic nursing and patient-centred care in home nursing affected by external framework conditions such as the coordination reform and governance principles inspired by New public management?

Method: This is a literature study based on the systematic approach of mainly qualitative research articles, nursing theory of Kari Martinsen, as well as non-fiction, professional ethical guidelines, laws, regulations and my own experiences.

Result: The studies refer to several challenges such as time pressure, understaffing and increased focus on results, which contributes to the fact that holistic nursing and patient-centred care is difficult to perform to a large enough extent.

Conclusion: The organisation of home care can threaten holistic nursing and patient care. Economic governance principles are increasingly prioritised. Thus, questions can be asked about whether the current organization has the right focus on the patient. Taking into account the studies, I also question whether primary care has the necessary resources and whether current resources are given priority for the performance of holistic nursing and good patient care.
