Effekten av cochlea implantat på taleforståelse og livskvalitet hos eldre med alvorlig til døvt postlingualt hørselstap: En systematisk gjennomgang av forskningslitteraturen publisert i perioden 2005-2020

Abstract

Denne bacheloroppgaven har undersøkt hvordan cochlea implantat (CI) påvirker taleforståelse og livskvalitet hos eldre med alvorlig til døve postlinguale hørselstap. Dette ble besvart ved bruk av en litteraturstudie med nyere relevant forskning. Det ble gjennomført en systematisk søkestrategi blant databasene PUBMED og MEDLINE. Søkeordene i første omgang var: ((elderly AND (cochlear implant)) AND (speech perception). Søkeordene i andre omgang var: ((elderly AND (cochlear implant)) AND (quality of life). I MEDLINE var søkeordene: elderly AND cochlear implantation AND «quality of life». Antall treff ved databasene var 1926, hvor det etter fjerning av duplikat var 1695 resterende artikler, ut fra de resterende artiklene var det 13 som ble inkludert ved denne litteraturstudien. Av disse tok 5 for seg taleforståelse, og 8 taleforståelse og livskvalitet. Det var 8 av artiklene som undersøkte taleforståelse i stille omgivelser blant eldre fra pre- og postoperativt, alle fant signifikant bedring. En artikkel fant signifikant bedre av taleforståelse i støyende omgivelser fra pre- til postoperativt hos eldre, en annen viste bedring av score fra pre- til postoperativt. Det var 4 studier som tok for seg livskvalitet fra pre- til postoperativt hos eldre. Tre av disse fant signifikant bedring, og én fant en bedring. To artikler undersøkte sammenhengen mellom taleforståelse og livskvalitet hos eldre med CI, hvor det ble funnet en sammenheng. Det ble ved denne litteraturstudien konkludert med at taleforståelse og livskvalitet bedret seg som følge av CI hos eldre, men at det er behov for forskning ved flere punkter. Det var ikke nok data for å konkludere om det var en sammenheng mellom taleforståelse og livskvalitet.

The aim of this study was to examine the effect of cochlear implant (CI) on speech perception and Quality of Life (QoL) in elderly with severe to profound postlingual hearing loss. A systematic review was conducted to find recent published research. The databases PUBMED and MEDLINE were used. In PUBMED the searches were: 1) ((elderly AND (cochlear implant)) AND (speech perception), 2): ((elderly AND (cochlear implant)) AND (quality of life). In MEDLINE the searches were: 1) elderly AND cochlear implantation AND «quality of life». The databases had 1926 hits. After removing duplicates, there were 1695 remaining articles. Thirteen of these were chosen for this review. Speech perception was researched in five articles, and eight researched speech perception and QoL. Scores were significant when comparing pre- and postoperative speech perception score in quiet environments. In noisy environments, the results were inconsistent; one article found a significant score while a second article found a better score that was not significant. When QoL was researched among four articles, three found a significant difference in postoperative scores, while the remaining found a better score. There were two articles that successfully found correlations between speech perception and QoL. Articles researching speech perception in quiet environments among elderly from pre- to postoperative had significant scores. Speech perception in noisy was significantly better among elderly from pre- to postoperative in one article, and another article found better scores. Four articles researched QoL among elderly from pre- to postoperative, three of which found significant better scores, while the fourth was not significant. There were two articles that successfully found significant correlations between speech perception and QoL. This literature study concludes that elderly gained better perception and QoL after receiving CI. There was not enough data to conclude whether there is a relationship between speech perception and QoL.
