Mot selvstendighet i transisjonsprosessen

Abstract

Innledning: Artikkelen tar for seg sykepleieres erfaring og opplevelse med et standardisert pasientforløp for ungdom mellom 16-25 år ved revmatologisk avdeling og hvilke innvirkninger det har hatt på overgangsomsorgen. Simultant belyser artikkelen hva sykepleierne vektlegger i konsultasjoner med den aktuelle pasientgruppen. Metode: For å undersøke hva sykepleiere vektlegger i møte med ungdom og hvordan pasientforløpet har fungert i praksis ble det gjennomført tre kvalitative semistrukturerte intervju med sykepleiere som hadde erfaring fra feltet. Intervjuene ble gjennomført i mars 2018 og analysert ved hjelp av prinsippene for systematisk tekstkondensering. Resultater: Resultatene viser at ungdomsforløpet har gjort det enklere å veilede til selvstendighet, tilpasse informasjon, skape en god relasjon og gi kontinuitet i behandlingen. Selvstendighet og tilrettelagt informasjon var også noe alle vektla under konsultasjonene. Ingen av informantene var usikre på om ungdomsforløpet har hatt en positiv innvirkning på overføring fra pediatrisk- til voksen avdeling. Konklusjon: Studien har vist at sykepleiere ved revmatologisk avdeling vektlegger å veilede unge til å bli selvstendige voksne mennesker gjennom tilpasset informasjon og god relasjonsbygging. Sykepleierne er enige om at ungdomsforløpet har bidratt til kontinuitet, tillit og trygghet. Enda fins det flere spørsmål som kan besvares med videre forskning; om transisjonsprosessen starter tidlig nok, om compliance og skriftlig informasjon har en sammenheng og hvordan ungdomsforløpet har fungert fra et pasientperspektiv.

Introduction: The objective of this article is to identify nurses' experiences with a standardized transition program for young people between the ages of 16-25 with rheumatological diseases and the effects this has had on transitional care. The article simultaneously illustrates what nurses emphasize in consultations with adolescents. Method: In order to investigate what nurses emphasize in consultations with adolescents and how the transitional care program has been working for the last four years, three qualitative semistructured interviews were carried out interviewing nurses who had experience with the patient group and the transitional program. The interviews were conducted in March 2018 at St. Olavs Hospital and analyzed using the principles of systematic text condensation. Results: The results show that the transitional program has made it easier to learn adolescents self- management skills, give understandable information, create a trusting relationship and provide continuity in the treatment. Every informant said they were sure that the transitional program has had a positive impact on the transition from the pediatric to the adult department. They also expressed that self- management skills and customized information is essential in consultations with adolescents. Conclusion: The project has shown that nurses at the rheumatological department emphasize helping young people become independent adults through customized information and building a trusting relationship in consultations with adolescents. At the same time, there is no doubt that the transitional care program has contributed to continuity, trust and security for the young patients. Nevertheless, there are still potential of improvement and questions that can be answered with further research; if the transitional process starts at the right time, if compliance and written information has a connection and what the patients think of the transitional care program.
