Background: The nurse communicates at all times with all patients, it is important for the nurse to be good at communicating to convey and understand patients' message. Good communication can lead to safety for the patient, which can ensure user and lead to patient-safety. The aim of this study is to identify the challenges that nurses face meeting with aphasia patients to ensure autonomy. Purpose: Examine how nurses can help to safeguard the autonomy of patients with aphasia after stroke, by using communication. Method: Literature study. 7 articles have been included in this study by systematic search for scientific research papers. Keywords that have been used: Aphasia, stroke patients, communication, stroke, care, autonomy, nursing. Results: Summary of the results is presented in 4 main themes, since the articles shows several similarities. Communication strategies, the patient's experience, group affiliation and meaningful activities as well as nurses' knowledge and attitudes are topics summarized and discussed. Conclusion: The nurse should emphasize the use of several different communication strategies, which are adapted to each patient. In order to exercise this nurses have to have enough knowledge about different strategies, and when they are supposed to be used. The nurse should speak slowly, be able to give the patient time to respond and include the patient in conversation. Picture tools can also be used as supplement to the other communication strategies. By using several different methods of communication and control questions, the nurse understand whether the patient has understood the message and what the patient conveys as important. This will provide a good basis for the nurse to attend to patient autonomy.nb_NO
