dc.description.abstractSummary: In 1999, patient education became legislated as one of the four main tasks of Norwegian hospitals. Today, patient education resource centres are to be found at all Norwegian hospitals, committing themselves to an ideology where users and professionals should be equal. This thesis investigates how patient education as equal collaboration between users and professionals can flourish within the health care sector. The investigation is done through four articles that investigate the intra- and extra-hospital accountability of user involvement at individual and organisational level. The investigation is built upon the distinction between system and Lifeworld, the distinction between experiential knowledge and professional knowledge, organisational decoupling and hypocrisy, and the plasticity of boundary objects. The study is ethnographic and explorative. The main instrument for data-collection is in-depth interviews. Stepwise Deductive Induction is used in the analysis and in the theory development. When the four articles are brought together, they demonstrate that it is crucial to the accountability of patient education that the concept of equality is maintained in many and diverse ways. These ways are only loosely coupled and are ontologically diverse. The diversity produced through decoupling at the boundaries sustain accountability because equality is undefined within the system. By being tightly bound to the Lifeworld, the meaning and truthfulness of equality can be maintained by health professionals that render the users equal to themselves. Patient education as equal collaboration between users and professionals is therefore highly dependent upon both savvy users and savvy professionals, that bend the system by being human beings.nb_NO
dc.description.abstractSammendrag: I 1999 vart opplæring av pasientar og pårørande ei av sjukehusa sine fire hovudoppgåver. I dag kan ein finne lærings- og meistringssenter, einingar som har eit særleg ansvar for å ta hand om denne oppgåva, på alle norske sjukehus. Ideologien er at erfarne brukarar og helsearbeidarar skal vere likestilt i planlegging, gjennomføring og evaluering av opplæringstiltaka. Denne avhandlinga undersøkjer vilkåra for denne likestillinga. Avhandlinga samlar fire artiklar. Desse studerer den interne og eksterne accountabiliteten til brukarmedverknaden i opplæring av pasientar og pårørande på både individuelt nivå og organisasjonsnivå. Skiljet mellom system og Livsverd, skiljet mellom erfaringsbasert brukarkunnskap og profesjonell kunnskap, organisasjonsbasert frikopling og plastisiteten til grenseobjekt er viktige verkty i analysen. Studien er etnografisk og eksplorativ. Hovudinstrumentet for datainnsamling er djupneintervju. Stegvis deduktisk induksjon er brukt i analyse og teoriutvikling. Funna i dei fire artiklane viser at det er avgjerande for accountabiliteten til opplæring av pasientar og pårørande at likstilling mellom brukar og helsepersonell vert skapt og gjennomført på mange ulike måtar, og at desse ulike måtane berre er lausleg kopla og dei er ontologisk ulike. Variasjonen som frikoplinga skaper på grensene mellom einingar skaper accountabilitet fordi likestilling ikkje er definert i systemet. Ved å vere tett kopla til Livsverda, kan meininga og sannferdet til likestilling bli skapt og haldt ved like av helsearbeidarar som gjer brukarar likeverdige dei sjølve. Opplæring av pasientar og pårørande som likeverdig samarbeid mellom brukarar og helsearbeidarar avheng såleis av både kløktige helsearbeidarar og kløktige brukarmedverkarar, som bøyer reglane i systemet for å kunne jobbe etter mellommenneskelege reglar.nb_NO
