dc.description.abstractEnglish summary Depression and anxiety in clinically representative therapy This thesis represents an initiative to strengthen research on patients and treatments in routine mental health care settings. It includes three published papers. Using a meta-analytic strategy in the first paper, it was found that very few Nordic studies have investigated the effectiveness of clinically representative treatment for adults. The results indicate that evidence-based treatments could be more effective than current routine praxis. However, the included studies varied considerably in their description of treatment delivered, leaving the results to be interpreted cautiously. The study's main contribution is that it highlights the need for more rigorous studies within the routine care setting. The thesis also explored the reliability and validity of two of the most commonly used instruments measuring symptoms of depression (Patient Health Questionnaire-9; PHQ-9), and anxiety (Generalized Anxiety Disorder scale-7; GAD-7). This second study used data from adult outpatients treated at Nidaros DPS, a local community mental health care center. The results suggest that the PHQ-9 and GAD-7 can be recommended for use in clinical settings and that they are related to functional impairment. The PHQ-9 and GAD-7 were also used to investigate possible subgroups of patients in routine care in the third paper. Three groups were found: anxiety and somatic depression (33%), mixed depression and anxiety (40%), and cognitive depression (27%). The anxiety and somatic depression group was associated with older age, being single and on sick leave, higher probability of depressive-, anxiety-, and comorbid disorders, having more appointments, and higher functional impairment. Better knowledge of naturally occurring mental health treatment could help inform decision-makers, researchers, mental health workers, and patients. This thesis provides insights into the multifaceted mental health praxis and stresses the importance of continuing to research treatment of patients in routine care settings.en_US
dc.description.abstractNorsk sammendrag Depresjon og angst i ordinære psykisk helsetjenester Denne doktoravhandlingen representerer et initiativ for å styrke forskning på pasienter og behandlinger i ordinære psykisk helsetjenester. Den inkluderer tre publiserte artikler. Gjennom å bruke en metaanalytisk prosedyre i den første artikkelen ble det funnet at svært få studier har undersøkt effektiviteten av nordisk klinisk representativ behandling for voksne. Videre indikerer resultatene at evidensbasert behandling kan være mer effektiv enn dagens praksis. Imidlertid varierte de inkluderte studiene betydelig i beskrivelsen av behandlingen, slik at resultatene må tolkes med forsiktighet. Studien synliggjør behovet for mer omfattende og nøyaktige studier av ordinære psykisk helsetjenester. Avhandlingen utforsket også reliabiliteten og validiteten til to av de mest brukte instrumentene som måler symptomer på depresjon (Patient Health Questionnaire-9; PHQ-9) og angst (Generalized Anxiety Disorder scale-7; GAD-7). Artikkel to brukte data fra voksne polikliniske pasienter ved Nidaros DPS. Resultatene støtter at PHQ-9 og GAD-7 kan anbefales for bruk i kliniske miljøer og at de er relatert til funksjonssvikt. Angst- og somatisk depresjonsgruppen var assosiert med høyere alder, å være singel, sykemeldt, høyere sannsynlighet for depressive, angst- og komorbide lidelser, ha flere behandlingstimer og høyere funksjonsnedsettelse. Bedre kunnskap om ordinær psykisk helsebehandling kan bidra til å informere beslutningstakere, forskere, psykisk helsearbeidere og pasienter. Avhandlingen gir et innblikk i den mangefasetterte psykiske helsepraksisen og understreker viktigheten av å fortsette å forske på behandling av pasienter i ordinære psykisk helsetjenester.en_US
