dc.description.abstract | I denne oppgaven ble det undersøkt om metoden for kvantifisering av fritt valproat i serum
kan optimaliseres med tanke på effektivitet og sammenlignbarhet. Valproat er et legemiddel
som brukes i behandling av epilepsi og bipolar lidelse, og detekteres ved hjelp av UPLCMS/MS. Intensivpasienter har økt risiko for å få lavere nivå av serumalbumin, derfor er
terapikontroll av fritt valproat i serum hos denne pasientgruppen viktig for å unngå overdose
og forgiftning. Formålet med oppgaven er å undersøke om konsentrasjonen av fritt valproat
påvirkes under prøveopparbeidelsen ved filtrering i romtemperatur versus kroppstemperatur.
Det ble gjennomført en enkel validering som innebar bestemmelse av nøyaktighet og
presisjon av fritt og totalt valproat i kontroller, sammenlignende laboratorieprøver og
pasientprøver. I tillegg ble det utført en metodesammenligning der avvik mellom metodene
ble undersøkt ved ulik filtreringstemperatur.
Resultatene viste at presisjon og nøyaktighet for de interne kontrollene var innenfor
akseptkriteriene, som henholdsvis var CV ≤ 10,0% for repeterbarhet, CV ≤ 15,0% for
reproduserbarhet og avvik ≤ 15,0% for nøyaktighet. Repeterbarheten for totalt valproat
varierte fra 3,1-5,7% og for fritt valproat varierte den fra 2,8-4,0%. Reproduserbarhet for
totalt valproat varierte fra 5,5-7,9% og for fritt valproat varierte denne fra 3,5-9,3%.
Nøyaktighet for totalt valproat varierte fra -2,7-2,7%. For de sammenlignende
laboratorieprøvene var krav til nøyaktighet satt til |Z| ≤ 2, der de utregnede Z-verdiene varierte
fra -1,4-2,1 for fritt valproat og for totalt valproat fra 2,8-3,6. CV ble utregnet til 10,0% for
fritt valproat og for totalt valproat utregnet til 1,5%. Metodesammenligningen viste at det var
en proporsjonal og konstant forskjell mellom metodene, der filtrering i romtemperatur ga i
gjennomsnitt 12,6% lavere konsentrasjon.
Undersøkelsen i sin helhet indikerte at prøveopparbeidelsen av valproat kan gjennomføres ved
filtrering i romtemperatur. Denne metoden gav resultater som var lavere enn ved filtrering i
kroppstemperatur. Samtidig ble metoden effektivisert og resultatene ble mer sammenlignbare
med andre laboratorier, da flere filtrerte i romtemperatur fra før. | |
dc.description.abstract | In this study, an examination was conducted to optimize a method for quantifying free
valproate in serum, with a focus on enhancing efficiency and comparability. Valproate, a
medication utilized in the treatment of epilepsy and bipolar disorder, is detected through
UPLC-MS/MS analysis. Given the elevated risk of decreased albumin levels among critical
care patients, it is crucial to monitor levels of valproate in serum for this patient group to
avoid overdose and poisoning. The purpose of this study is to examine if the concentration of
free valproate in serum is influenced by the temperature at which filtering occurs during
sample processing, comparing room temperature to body temperature.
A simplified validation was implemented, which included the determination of accuracy and
precision of free and total valproate in controls, comparative laboratory samples and patient
samples. Additionally, a method comparison was conducted to evaluate deviation between the
two methods.
The results showed that the precision and accuracy of internal controls met the acceptance
criteria, which was respectively CV ≤ 10,0% for repeatability, CV ≤ 15,0% for reproducibility
and deviation ≤ 15,0% for accuracy. Repeatability ranged between 3,1-5,7% for total
valproate and between 2,8-4,0% for free valproate. The reproducibility ranged between 5,5-
7,9% for total valproate and for free valproate it varied between 3,5-9,3%. Accuracy for total
valproate varied between -2,7-2,7%. The comparative laboratory samples were assessed with
accuracy criteria set at |Z| ≤ 2. The calculated Z-values ranged from -1,4-2,1 for free valproate
and 2,8-3,6 for total valproate. CV was determined to be 10,0% for free valproate and 1,5%
for total valproate. Method comparison revealed a consistent and proportional difference
between the two methods, with filtering at room temperature resulting in an average
concentration that was 12,6% lower.
This study uncovered that processing valproate samples through room temperature filtering
yields lower results compared to body temperature filtering. These results also showed a more
comparable method with other laboratories, as the majority already use the filtering method at
room temperature. | |
