Klinisk observasjonskompetanse i kommunehelsetjenesten. Helsepersonell sine erfaringer med implementering og bruk av ABCDE, NEWS og ISBAR.
Abstract
Bakgrunn: En tredjedel av unødige dødsfall skyldes manglende eller sen oppdagelse av forverret tilstand, mangelfull observasjon og svikt eller misforståelser i kommunikasjon. Økt observasjons- og medisinskfaglig kompetanse og sikker kommunikasjon vil kunne øke pasientsikkerheten. Helsedirektoratet anbefaler at observasjonskompetanse bygger på ABCDE-prinsippene, skåringsverktøyet NEWS og kommunikasjonsverktøyet ISBAR. Etter at samhandlingsreformen ble iverksatt behandles og følges stadig flere pasienter opp i kommunehelsetjenesten. For å kunne ivareta disse pasientene kreves det at sykepleiere i kommunale helse- og omsorgstjenester innehar stor kompetanse.
Hensikt: Undersøke hvilke erfaringer helsepersonell i kommunehelsetjenesten har knyttet til implementering og bruk av ABCDE, NEWS og ISBAR.
Metode: Det ble brukt en kvalitativ tilnærming for å belyse tema og forskningsspørsmål. Det ble gjennomført 4 intervju med til sammen 9 deltakere. En semistrukturert intervjuguide ble utarbeidet og brukt under intervjuene og intervjuene ble tatt opp med lydopptaker. For analyse av data ble det brukt en tematisk analysemetode.
Resultat: Intervjuene resulterte i fire hovedtema: 1) Forhåndskunnskaper om verktøyene og årsaker for innføring, 2) Viktigheten av opplæring, 3) Bruk av verktøyene, 4) Erfaringer av endringer etter innføring av verktøyene

Increased observational and medical professional expertise and safe communication may enable an increase in patient safety. The Norwegian Directorate of Health recommends that observation skills are based on the ABCDE principles, the NEWS screening tool and the ISBAR communication tool. After the implementation of the cooperation reform more patients are being treated and followed up in the municipal health care services. In order to take care of these patients, it’s required that nurses in the municipal health care services have high competence.
Aim: To investigate the experiences of health personnel in the municipal health care services relating to the implementation ang use of ABCDE, NEWS and ISBAR.
Method: A qualitative approach was used to enlighten the themes and research questions. 4 interviews were conducted with a total of 9 participants. A semi-structured interview guide was prepared and used during the interviews. The interviews were recorded by an audio recorder. For analysing the data, a thematic method was used.
Result: The interviews resulted in four main themes: 1) Prior knowledge of the tools and reasons for introduction, 2) The importance of instruction, 3) Use of the tools, 4) Experiences of changes after the introduction of the tools
