Tittel: Hvordan kan sykepleier i et tverrprofesjonelt team, bidra til at KOLS pasienter kan bo mest mulig hjemme?

Bakgrunn: En økendeutfordring i helsevesenet er samtidig mangel på helsepersonell og økende antall eldre med komplekse sykdomsbilder. Dette kombinert med et økende fokus på at behandling skal skje på lavest mulig nivå gjør at flere komplekse pasienter behandles i hjemmet. Dette krever gode tiltak og godt arbeid i team spesielt for sykepleierne som skal jobbe både med pasienten og i det tverrprofesjonelle temaet.

Hensikt: Studiens hensikt er å belyse den sammensatte problematikken og hvilke tiltak som kan bevare KOLS pasienten i hjemmet og hvilke tiltak og rolle som tilfaller en sykepleier hos denne pasientgruppen. Problemstilling: Hvordan kan sykepleier i et tverrprofesjonelt team, bidra til at kols pasienter kan bo mest mulig hjemme?

Metode: Studien er en strukturert litteraturstudie der søket ble utført i Cinahl complete og Medline gjennom EBSCOhost. Dette resulterte i 7 artikler, både kvalitative og kvantitative. Disse ble tematisk analysert med Aveyard´s tematiske analyse.

Resultat: Gjennom analysen laget vi to hovedtemaer med seks totale undertemaer:

Tema 1: Pasientens subjektive situasjon og sykdomsforståelse Tema 2: Hjelpe KOLS-pasienter å være hjemme mest mulig

Konklusjon: Denne litteratur-studien viser en gjennomgående positiv trend mot færre innleggelser og akuttmottak besøk, forbedret livskvalitet, forbedret sykdom og medsinsforståelse hos KOLS-pasienter som blir innrullet i en intervensjon. Det kreves mer forsking på dette området for å fastslå hvilken intervensjon som er mest kostnadseffektiv og ressurseffektiv samt hvilke intervensjoner som funker best på de forskjellige stadiene med KOLS

Abstract

Title: How can nurses in an interprofessional team contribute to enabling COPD patients to stay at home as much as possible?

Background: An increasing challenge in the healthcare system is the simultaneous shortage of healthcare professionals and the growing number of older adults with complex medical conditions. Combined with a focus on providing treatment at the lowest level of care possible, this has led to more complex patients being treated at home. This requires effective measures and teamwork, especially for nurses who work both directly with the patients and within the interprofessional team.

Objective: The aim of this study is to shed light on the complex issues involved and identify interventions that can help keep COPD patients at home, as well as the role of nurses in this patient group.

Research question: How can nurses in an interprofessional team contribute to enabling COPD patients to stay at home as much as possible?

Method: The study is a structured literature review conducted using Cinahl Complete and Medline via EBSCOhost. This resulted in 7 articles, both qualitative and quantitative, which were thematically analyzed using Aveyard's thematic analysis.

Results: Through the analysis, we identified two main themes with a total of six sub-themes: Theme 1: The patient's subjective situation and understanding of the disease. Theme 2: Assisting COPD patients in staying at home as much as possible.

Conclusion: This literature review consistently demonstrates a positive trend towards fewer hospital admissions and emergency department visits, improved quality of life, improved disease management, and enhanced medication understanding among COPD patients enrolled in different interventions. Further research is needed in this area to determine the most cost-effective and resource-efficient interventions, as well as which interventions work best at different stages of COPD..
