Hvordan kan sykepleier etablere en terapeutisk relasjon til unge pasienter som lider av Anorexia nervosa?

Abstract

Bakgrunn: Anorexia nervosa er en alvorlig spiseforstyrrelseslidelse. En terapeutisk relasjon er et viktig aspekt i behandling av disse pasientene. En utfordring ved behandlingen er å skape denne relasjonen grunnet lidelsens kompleksitet.

Hensikt: Oppgavens hensikt er å belyse gjennom litteratur, de områdene som ligger til grunn for at sykepleier skal kunne skape en terapeutisk relasjon til en ung pasient.

Metode: Litteraturstudien inkluderer hovedsakelig åtte relevante kvalitative forskningsartikler som er hentet fra databasene Cinahl Complete og Pubmed. Disse artiklene er knyttet til temaet, og ble identifisert ved hjelp av søkeordene: Anorexia nervosa, adolescent, nurses/nursing care, og therapeutic alliance.

Resultat: Analysen avdekker fire temaer: «Tillit og allianse i utformingen av relasjon», «skillet mellom individ og lidelsen», «kunnskap og kompetanse», «mellommenneskelige forhold».

Konklusjon: Litteraturstudien utforsker faktorer som er betydningsfullt for sykepleiere i utviklingen av en terapeutisk relasjon. Den setter søkelys på elementer ved å skape en relasjon i tilfriskning hos pasienter med Anorexia nervosa. Gjennom dette belyses områdene sykepleierne må ta hensyn til. Dette inkluderer at det innledende møte med pasienten skal baseres på å skape et tillitsfullt forhold og danne en allianse uten bruk av makt.

Studien fremhever betydningen av økt innsikt og kunnskap om lidelsen, og pasientens adferd som avgjørende for å skille mellom pasienten og lidelsen som fenomen. For å kunne bygge videre den terapeutiske relasjon må sykepleier benytte seg av sine mellommenneskelige ferdigheter, hvor empati og respekt fremheves. Det er derfor essensielt at sykepleieren ikke lar sine forutinntatte meninger og oppfatninger om lidelsens påvirke deres tilnærming.

Background: Anorexia nervosa is a serious eating disorder. A therapeutic relationship is an important aspect in the treatment of these patients. A challenge in the treatment is to create this relationship due to the complexity of the disorder.

Purpose: The purpose of the assignment is to shed light with literature on the areas that are the basis for a nurse to be able to create a therapeutic relationship with a young patient.

Methods: The literature study mainly uses qualitative research studies. The literature study contains eight research articles, which are related to the theme. The databases Cinahl Complete and Pubmed were used in the selection of the articles. The article was found using the search terms: Anorexia nervosa, adolescent, nurses/nursing care and therapeutic alliance.

Result: The analysis reveals four themes: "Trust and alliance in the design of relationships", "distinction between the individual and the disorder", "knowledge and competence", "interpersonal relationships".

Conclusion: The literature study explores key factors that are of importance to nurses in the development of a therapeutic relationship. It focuses on elements by creating a relationship in the recovery of patients with Anorexia nervosa. Through this, the areas that the nurses must consider are highlighted. This includes that the initial meeting with the patient must be based on creating a trusting relationship and forming an alliance without the use of force.

The study highlights the importance of increased insight and knowledge about the disorder and the patient's behavior as crucial for distinguishing between the patient and the disorder as a phenomenon. To build on the therapeutic relationship, nurses must make use of their interpersonal skills, where empathy and respect are highlighted. It is therefore essential that the nurse does not allow their preconceived opinions and perceptions about the disorder to influence their approach.
