På St.Olavs Hospital ved enheten for serologi ved avdeling for medisinsk mikrobiologi benyttes per dags dato den manuelle metoden Line Blot fra recomLine. Metoden benyttes for identifisering av IgM antistoff mot bakterien Borrelia Burgdorferi. Line Blot krever mye tid og ressurser. Avdelingen ønsker derfor å bytte Line Blot ut med det nye instrumentet VirClia levert av Vircell. For at Line Blot skal kunne byttes ut med VirClia må den nye metoden ikke være mindreverdig nåværende metode. Hensikten med dette bachelorprosjektet er å sammenlikne de to metodene og undersøke avvik i prøveresultat mellom dem.
Totalt 59 prøver ble analysert ved bruk av begge metodene. Vi analyserte fem ulike prøvegrupper for å undersøke VirClia i ulike situasjoner som muligheten for kryssreaktivitet, og falske positive resultater fra screeningmetoden på instrumentet Liaison XL.
I gruppen med positive syfilisprøver for testing av kryssrektivitet, viser resultatene til at VirClia er mest spesifikk da den ikke ga utslag på noen av prøvene. Likevel viser resultatene fra kontrollprøvene og prøvene som tidligere ble negative på Liaison at VirClia gir et avvik på 12%. De to siste prøvegruppene ga sprikende resultater og diskuteres nærmere under «4.0 Diskusjon».
Det er vanskelig å trekke en konklusjon om Line Blot kan byttes ut med VirClia basert på det begrensede antallet prøver som ble brukt i prosjektet. Med kun 59 prøver totalt og noen prøvegrupper som ikke inneholder flere enn seks prøver, vil ett avvik utgjøre en stor prosentandel. Derfor er ikke resultatene fra dette prosjektet det eneste som bestemmer om byttet skal gjennomføres. Vi ville anbefalt å benytte et større prøvevolum for å få et bedre inntrykk av VirClia i forhold til Line Blot. The unit of serology in the department for medical microbiology at St.Olavs Hospital are currently using the manual method Line Blot from recomLine for identification of IgM antibodies against Borrelia. This method is time consuming, and the department wants to replace it with the new instrument VirClia from Vircell. To replace the current method, the new method has to be not inferior to the old one. The purpose of this Bachelor’s thesis is to compare VirClia to Line Blot and investigate discrepancies between these two methods.
A total of 59 samples were analysed using both methods. We analysed five different sample groups to test VirClia in different situations like the possibility of cross reactivity, and false positive test results from the screening method on the instrument Liaison XL.
In the group of positive syphilis samples to test cross reactivity, VirClia proves to be the most correct as it did not detect enough Borrelia antibodies to give any positive results. However, in the group of control samples, and the samples that previously tested negative on the screening method, there was a discrepancy of 12%. The two last sample groups gave mixed results and are discussed more closely under the discussion chapter “4.0 Diskusjon”.
It is difficult to draw a conclusion on whether VirClia can replace Line Blot based on such a limited number of samples. With only 59 samples in total, and some sample groups containing only six samples, one discrepancy will amount to a relatively large percentage. This is why the numbers in this paper is not the only thing deciding whether or not VirClia replaces Line Blot. We would recommend that more samples should be analysed before drawing a conclusion.
