Sammendrag
Spredning av antibiotikaresistente bakterier har blitt et økende problem i verden, og meticillinresistent Staphylococcus aureus (MRSA) er en forekommende antibiotikaresistent bakterie. Foreløpig er forekomsten av MRSA i Norge lav, men det er likevel viktig å forhindre spredning av MRSA. S. argenteus er en annen Staphylococcus-art som også kan ha inneha meticillinresistente gener og omtales da som MRSArg. Tidligere har S. argenteus blitt feilidentifisert som S. aureus, men defineres nå som en egen art tilhørende S. aureus-komplekset. Det er fortsatt lite kunnskap om S. argenteus, men da bakteriearten innehar flere av de samme virulensgenene som S. aureus og virker til å ha likt patogent potensiale, er det hensiktsmessig å også utføre påvisning og screening av MRSArg for å overvåke resistensutvikling og spredning.
Bachelorprosjektet ble utført ved Nasjonalt referanselaboratorium for MRSA ved Avdeling for medisinsk mikrobiologi ved St. Olavs hospital i Trondheim. Hensikten med prosjektet var å evaluere metoder for vekst, påvisning og screening av MRSArg.
27 MRSArg-bakteriestammer fra frysearkivet i stammebanken til MRSA-referanselaboratoriet ble benyttet for undersøkelse av kromagarer og anrikningsbuljonger som metoder for vekst og dyrkning og MALDI-TOF MS og hurtig-PCR på GeneXpert som metoder for identifisering, av MRSArg.
Resultatene viste at alle kromagarene er ideelle for dyrkning av MRSArg og at kromagaren MRSM fra BioMérieux Clinical Diagnostics var den som viste best sensitivitet for bakteriearten. Anrikningsbuljongen PHMB ga høyest cellekonsentrasjon ved anrikning av MRSArg. For identifikasjonsmetodene viste resultatene at MALDI-TOF MS ga riktig identifikasjon til 21 av 27 MRSArg-bakteriestammer og hurtig-PCR på GeneXpert påviste resistensgener hos MRSArg-bakteriestammene og ga de identifikasjon som MRSA og SA.
Det kan konkluderes med at flere av metodene kan brukes for vekst, påvisning og screening av MRSArg, men noen av metodene bør benyttes i kombinasjon med hverandre for å kunne med sikkerhet identifisere MRSArg. The spread of antibiotic resistant bacteria has become an increasing problem in the world, and methicillin-resistant Staphylococcus aureus (MRSA) is an occurring antibiotic resistant bacterium. Currently, the incidence of MRSA in Norway is low, but it is still important to prevent the spread of MRSA. S. argenteus is another Staphylococcus species that can also have bacterial strains with methicillin-resistant genes and is then referred to as MRSArg. S. argenteus has formerly been misidentified as S. aureus but is now defined as a separate species belonging to the S. aureus complex. There is still little knowledge about S. argenteus, but since the bacterial species possesses several of the same virulence genes as S. aureus and seems to have similar pathogenic potential, it is appropriate to also carry out detection and screening of MRSArg to monitor the development and spread of resistance.
The bachelor's project was carried out at the National Reference Laboratory for MRSA at the Department of Medical Microbiology at St. Olav's Hospital in Trondheim. The purpose of the project was to evaluate methods for growth, detection and screening of MRSArg.
27 MRSArg bacterial strains from the frozen archive in the strain bank of the MRSA reference laboratory were used for the investigations of solid and liquid growth media as methods for growth and cultivation and MALDI-TOF MS and rapid PCR on GeneXpert as methods for identification of MRSArg.
The results showed that all the chromogenic media are ideal for the cultivation of MRSArg and that the chromogenic media MRSM, from BioMérieux Clinical Diagnostics, was the one that showed the best sensitivity for the bacterial species. The enrichment broth PHMB gave the highest cell concentration when enriching MRSArg. For the identification methods, the results showed that MALDI-TOF MS correctly identified 21 out of 27 MRSArg bacterial strains and rapid PCR on GeneXpert detected resistance genes in the MRSArg bacterial strains and gave them identification as MRSA and SA.
It can be concluded that several of the methods can be used for growth, detection and screening of MRSArg, but some of the methods should be used in combination with each other to be able to identify MRSArg with certainty.
