Ivaretakelse av pårørende til pasienter ved livets slutt

Abstract

NORSK: Introduksjon: Pasienter i livets sluttfase har et redusert behov for væske og ernæring. Det kan for mange pårørende være vanskelig å se at pasienten gradvis slutter å spise og drikke. På bakgrunn av dette kan mange pårørende ønske iverksettelse av væske- og ernæringsbehandling. Hensikt: Belyse hvilke faktorer som er av betydning for at sykepleier på best mulig måte kan ivareta pårørende til pasienter i livets sluttfase, der væske- og ernæringsbehandling anses å være nytteløs. Metode: Oppgaven er basert på litteraturstudie som metode. Usystematiske og systematiske søk er utført i følgende databaser: CINAHL Complete, Ovid Nursing Database, SweMed+, AgeLine, Medline og Proquest. Sentrale søkeord: End of life care, next of kin, life-prolonging treatment, nutrition, hydration. Hovedresultater: Det er viktig at sykepleier viser omsorg for pårørende. Helsepersonell har manglende rutiner ved informasjonsgivning til pårørende. Det avdekkes også at pårørende og helsepersonell har ulike oppfatninger knyttet til væske- og ernæringsbehandling. Pårørende har behov for informasjon om fysiologiske forandringer hos pasienten ved livets slutt, samt virkninger og bivirkninger av væske- og ernæringsbehandling. Konklusjon: Litteraturstudien belyser viktigheten av å ivareta pårørende til pasienter ved livets slutt. Dette innebærer blant annet at sykepleier gir pårørende informasjon om pasientens helsetilstand og den helsehjelp som ytes. Samtidig belyses viktigheten av at sykepleier informerer pårørende om fysiologiske forandringer hos den døende pasienten, samt om virkninger og bivirkninger av væske og ernæringsbehandling. Dette kan bidra til å skape en felles forståelse av at behandlingen som gis, er til det beste for pasienten.

ENGLISH: Introduction: Patients in the terminal stage have a reduced need for hydration and nutrition. It may be difficult for many relatives to realize that the patient gradually stops eating and drinking. On this basis, many relatives desire implementation of hydration and nutrition therapy. Aim: To clarify which factors that enables nurses to provide the best possible care for the relatives of patients in the terminal stage, when hydration and nutrition therapy is considered to have no effect. Method: The thesis is based on a literature review as a method. Unsystematic and systematic searches are conducted in the following databases: CINAHL Complete, Ovid Nursing Database, SweMed+, AgeLine, Medline and Proquest. The main keywords are: end of life care, next of kin, life-prolonging treatment, nutrition, hydration. Main results: It is important that nurses care for the relatives. Health professionals have inadequate routines concerning providement of information to the relatives. In this thesis, it is revealed that relatives and healthcare professionals have different perceptions about hydration and nutrition therapy. Relatives need information about physiological changes in the patient as life comes to an end, as well as effects and side effects of the hydration and nutrition therapy. Conclusion: This literature review highlights the importance of nurses caring for the relatives of patients at the end of life. This means that the nurse informs relatives about the patient's health status and the health care that is given. At the same time, the importance of the nurse informing relatives about physiological changes in the dying patient and the effects and side effects of hydration and nutrition therapy is emphasized. This might help to form a common understanding that the treatment that is given is in the patient’s best interest.
