dc.description.abstractBakgrunn Staphylococcus aureus-endokarditt er en alvorlig sykdom med høy mortalitet. Perifere venekatetre kan være en inngangsport for denne infeksjonen. På grunn av den høye forekomsten av perifere venekatetre i klinisk praksis kan disse derfor utgjøre en betydelig risiko for pasientene. Med dette utgangspunktet ønsket vi å gjøre en epidemiologisk studie på endokarditt forårsaket av Staphylococcus aureus ved St. Olavs Hospital fra 2013 til 2018, og spesielt vurdere perifere venekatetre som mulig årsak til Staphylococcus aureus-endokarditt. Metode Pasienter med Staphylococcus aureus-blodbaneinfeksjon er identifisert fra laboratoriedata i informasjonssystemet ved Avdeling for medisinsk mikrobiologi ved St. Olavs hospital. ICD-10-diagnosekodene I33.0 og I33.9 ble benyttet for å hente ut aktuelle pasienter med endokarditt. Data for aktuelle sykdomsepisode er samlet ved systematisk gjennomgang av pasientjournaler der endokarditt og blodbaneinfeksjon er påvist (totalt 67 tilfeller) fra 2013 til 2018. Resultater Det ble identifisert 661 pasienter med Staphylococcus aureus-blodbaneinfeksjon, av disse ble 62 inkludert i studien. Kun tre pasienter hadde dokumentert perifert venekateter i journal, 6,7 % vurdert til å ha dette som fokus. 37,1 % hadde ukjent fokus. Hyppigste fokus var osteomyelitt/septisk artritt (11,3 %), fulgt av IV-rusmisbruk (9,7 %) og fremmedlegeme (9,7 %). Alle pasientene ble undersøkt med transtorakal ekko, transøsofagal ekko eller begge i løpet av oppholdet. 13 pasienter hadde bekreftet vegetasjon med transtorakal ekko, 24 med transøsofagal ekko. Median fra innkomst til positiv ekko var 5,3 dager. Hypertensjon var hyppigste form for for komorbiditet (50 %). Emboli til CNS var hyppigste komplikasjon (24,2 %). Median alder på pasient var 71 år og 66,1 % av pasientene var menn. Total dødelighet for 30 og 90 dager var henholdsvis 21 % og 27,5 %. Konklusjon Endokarditt forårsaket av Staphylococcus aureus er forbundet med høy morbiditet og sykehusdødelighet. Denne retrospektive studien bekrefter flere funn fra lignende studier samt belyser utfordringen mangelfull journalføring gir ved retrospektiv forskning. Mer systematisk dokumentasjon i journal er nødvendig for å kunne vurdere fokus for infeksjon med større sikkerhet.
dc.description.abstractBackground Staphylococcus aureus endocarditis is a serious disease with high mortality. Peripheral venous catheters can be a gateway for this infection. Due to the high incidence of peripheral venous catheters in clinical practice, these may pose a significant risk for infection. With this in mind, we wanted to do an epidemiological study on endocarditis caused by Staphylococcus aureus at St. Olavs hospital from 2013 to 2018, and especially consider peripheral venous catheters as a possible cause of Staphylococcus aureus endocarditis. Method Patients with Staphylococcus aureus bloodstream infection have been identified from laboratory data in the information system at the Department of Medical Microbiology at St. Olav's Hospital. ICD-10 diagnostic codes I33.0 and I33.9 were used to retrieve current patients with endocarditis. Data for current disease episodes were collected by a systematic review of patient records where endocarditis and blood stream infection were detected (a total of 67 cases) from 2013 to 2018. Results 661 patients with Staphylococcus aureus bloodstream infection were identified, of whom 62 were included in the study. Only three patients had documented peripheral venous catheters in the medical record, 6.7% considered to have this as a focus. 37.1% had an unknown focus. The most common focus was osteomyelitis/septic arthritis (11.3%), followed by IV drug abuse (9.7%) and foreign body (9.7%). All patients were examined with transthoracic echocardiography, transesophageal echocardiography, or both during their stay. 13 patients had confirmed vegetation with transthoracic echo, 24 with transesophageal echo. The median from income to positive echo was 5.3 days. Hypertension was the most common form of comorbidity (50%). CNS embolism was the most common complication (24.2%). The median age of the patient was 71 years and 66.1% of the patients were men. Total mortality for 30 and 90 days was 21% and 27.5%, respectively. Conclusion Endocarditis caused by Staphylococcus aureus is associated with high morbidity and hospital mortality. This retrospective study confirms several findings from similar studies and highlights the challenge of inadequate record keeping in retrospective research. More systematic documentation in the medical record is necessary to be able to assess the focus for infection with greater certainty.
