Sykehuslegenes syn på samhandlingen med allmennlegetjenesten

Abstract

Denne masteroppgaven tar for seg hvordan sykehusleger i Norge opplever samhandlingen med allmennlegetjenesten og ulike utfordringer i samhandlingen før og etter innføringen av samhandlingsreformen. Det empiriske grunnlaget for oppgaven er surveyundersøkelser som er sendt ut til et representativt utvalg av norske sykehusleger i årene 2006 og 2016. Gjennom multiple regresjonsanalyser tar oppgaven for seg hvordan sykehusleger med ulik faglig bakgrunn oppfatter samhandlingen med allmennlegetjenesten, og hvordan de oppfatter fem ulike utfordringer som et problem for samhandlingen. De fem utfordringene som blir tatt for seg i denne oppgaven er tidspress i forbindelse med utskriving, manglende mottakskapasitet i kommunene, mangelfull informasjon i henvisninger fra fastleger og for sterk spesialisering blant sykehusleger. Oppgaven konkluderer med at sykehuslegene var mindre fornøyde med samhandlingen i 2016 sammenlignet med 2006, men det er ingen systematiske mellom de ulike avdelingene. Det konkluderes også med at utfordringene med tidspress i forhold til utskriving, manglende mottakskapasitet i kommunene og mangelfull informasjon i henvisninger fra leger er blitt et større problem, mens utfordringen om for sterk spesialisering blant sykehusleger er blitt mindre problematisk. Man ser også at leger som arbeider tettere med pasienter systematisk opplever utfordringene i samhandlingen som et større problem enn leger som ikke jobber like tett med pasienter.

This master thesis examines how physicians in Norwegian hospitals review the co-operation with general practitioners (GPs), and how they look at different challenges in the interaction before and after the introduction of the coordination reform. The empirical base of this master thesis is surveys sent out to a representative sample of Norwegian physicians working in hospitals in 2006 and 2016. Through multiple regression analysis the thesis addresses how physicians with different professional background perceive the interaction with GPs, and how they perceive five different challenges for the co-operation. The five challenges addressed in this thesis is “time pressure in hospital discharge”, “lack of reception capacity in the municipalities” “referral quality from GPs”, and “too strong specialization among hospital physicians”. The thesis concludes that physicians in Norwegian hospitals were less satisfied with the co-operation in 2016 compared to 2006, but there is no sign of systematic differences between the various departments. It also concludes that the challenges with “time pressure in hospital discharge”, “lack of reception capacity in the municipalities” and “referral quality from GPs” have become a greater problem, while “too strong specialization among hospital physicians” has become less problematic. The results also show that doctors who work more closely with patients experience the challenges in the co-operation as more problematic than those physicians who do not work as closely with patients.
