Hvordan påvirker isolering og smittevernutstyr kommunikasjonen mellom sykepleier og pasient?

Abstract

Bakgrunn: Med økende forekomst av smittsomme sykdommer og antibiotikaresistens kan bruken av smittevernutstyr og isolering i stadig økende grad bli en del av interaksjonen mellom sykepleier og pasient. For sykepleiere kan det å bruke verbal og nonverbal kommunikasjon i arbeidet være nødvendig for å sikre at sykepleien er faglig, etisk og juridisk forsvarlig. Dermed blir det nødvendig å undersøke hvilke konsekvenser smittevernutstyret har for kommunikasjonen. Hensikt: Hensikten med studien er å øke forståelsen av hvordan kommunikasjon mellom sykepleier og pasient påvirkes av isolering og smittevernutstyr. Metode: Oppgaven er en allmenn litteraturstudie. Strukturerte søk har identifisert syv relevante vitenskapelige forskningsartikler hvorav seks kvalitative og en triangulert kvantitativ-kvalitativ. Artiklene er kritisk vurdert ved hjelp av blant annet sjekklistene fra Folkehelseinstituttet. Analysen identifiserte fire nye hovedtemaer som kan belyse problemstillingen. Resultat: Litteraturstudien viser at isolering og smittevernutstyr påvirker både rammene for kommunikasjonen og selve kommunikasjonssituasjonen mellom sykepleiere og pasient. Merarbeid og tidspress gjør isoleringen i seg selv til en kommunikasjonsbarriere. Smittefrykt og utrygge rammer legger press på kommunikasjonen. Fysisk ubehag fra smittevernutstyr blir en distraksjon i kommunikasjonssituasjonen, samtidig som utstyret hindrer eller endrer særlig den nonverbale kommunikasjonen. Konklusjon: Konklusjonen viser at både arbeidsgiver og sykepleier må legge særlig til rette for å sikre en kommunikasjonspraksis som ivaretar isolerte pasienters behov. Arbeidsgiver må sørge for trygge rammer og god kommunikasjon med ansatte om rutiner, utstyr og kunnskapsgrunnlag. Sykepleier må være bevisst hvordan utstyret påvirker, og ta i bruk de nonverbale kanalene som fortsatt er tilgjengelige. Background: With an increase in risk of infectious disease outbreaks as well as prevalence of antibiotic-resistant pathogens, personal protective equipment (PPE) and isolation precautions may increasingly become part of the nurse-patient interaction. To nurses, verbal and nonverbal communication play a vital role in professionally, ethically and legally sound practice. There is a need for research examining the consequences of PPE and isolation on communication. Purpose: This study aims to increase understanding of how nurse-patient communication is affected by source isolation and nurses’ use of PPE. Method: Seven relevant articles identified through systematic searches were included. Articles were critically appraised using checklists from the Norwegian institute of Public Health. An analysis identified four new main themes that could illuminate the problem. Results: The findings demonstrate how source isolation and the use of PPE are affecting communication both directly and indirectly. Increased workload make the isolation a barrier in itself. Fear of contagion and uncertainty about guidelines puts strain on communication. Physical discomfort of wearing PPE becomes a distraction within the communication encounter, and also hinders or alters nonverbal communication. Conclusion: Measures must be taken by hospital management as well as by the individual nurses in order to ensure that communication practices are meeting patient needs. Safe working conditions and open lines of communication between management and staff regarding routines and equipment as well as the basis of knowledge behind it. Nurses must be aware of the effects of PPE on communication and adjust their communication, i.e. by making use of less affected communication channels.
