Hjemmedød for kreftpasienter

Abstract

Bakgrunn: I 2019 var det 1 354 som døde i private hjem. Flesteparten av pasientene som ønsket å dø hjemme, døde på en institusjon eller sykehus. Tilbudene i kommunene kan være med på å bestemme hvor kreftpasienten dør, men hjemmesykepleien og pårørende spiller en sentral rolle for at kreftpasienten skal få dø hjemme.

Hensikt: Hensikten med denne oppgaven er å belyse hvordan sykepleiere kan tilrettelegge for en hjemmedød for kreftpasienter.

Metode: Dette er en litteraturstudie hvor vi har brukt databasene CINAHL og MEDLINE for våre systematiske søk. Vi brukte søkeordene: «home death», «patient», «palliative care», «at home», «home care nursing», «cancer», «Norway», «family». Vi ble tipset om en artikkel som vi søkte opp i Idunn og inkluderte. Vi endte opp med fem kvalitative artikler.

Resultat: En god start kan skape tillit mellom sykepleierne, pårørende og pasienten. Tilstrekkelig informasjon er nødvendig for at hjemmesykepleierne kan forberede seg til å ta imot pasienten. Kontinuitet, koordinering og tilgjengelighet er viktig for at pasienten og pårørende skal føle seg ivaretatt og for at hjemmesykepleien skal føle at de strekker til. For at kreftpasienten skal kunne dø hjemme, er det essensielt at pårørende stiller opp og tar på seg mye ansvar. Hjemmesykepleien kan trenge pause fra pasienten og et godt støtteapparat rundt seg for å kunne fortsette å jobbe med palliative pasienter.

Konklusjon: Samhandling, kontinuitet og koordinering er viktig for at pasientens ønske skal oppfylles. Planleggingen av en hjemmedød bør starte tidlig og alle partene bør inkluderes fra starten. God informasjon og samhandling mellom partene er nødvendig for å kunne forberede hva som kan skje ved livets slutt.

Background: In 2019 it was 1 354 that died in their own home. Most of the patients who wanted to die at home, died in an institution or a hospital. The offers in the municipality can play a role in where the cancer patient dies, but the home care nurses, and the next-of-kin plays a key role so the cancer patient can die at home.

Aim: The aim of this study is to illuminate how nurses can facilitate for a home death for cancer patients. Method: This thesis is a literature study where the databases CINAHL and MEDLINE were used to implement systematic searches. Keywords used were: “home death”, “patient”, “palliative care”, “at home”, “home care nursing”, “cancer”, “Norway”, “family”. We got a tip about a study that we looked up in Idunn and included. We ended up with five qualitative studies.

Results: A good start can create trust between nurses, next-of-kin and the patient. Sufficient information is necessary so the homecare nurses can prepare to welcome the patient. Continuity, coordination and availability are important for the patient and next-of-kin to feel taken care of and the homecare nurses can feel they are reaching out. It is essential that next-of-kin lines up and take on a lot of responsibility so the patient can die at home. Homecare nurses can need a break from the patient and a good support system around them to continue in the job with palliative patients.

Conclusion: Collaboration, continuity and coordination is important so the patient’s wish can be fulfilled. Planning a home death should begin early and all parties should be included from the beginning. Good information and collaboration between the parties is necessary to prepare for what can happen in end-of-life-care.
