En sammenligning av to protokoller for bestemmelse av kontrastmengde til pasienter som gjennomgår CT-abdomen

Abstract

Problemstilling: Hvilke fordeler har en protokoll basert på kroppskomposisjon i forhold til en protokoll basert på vektkategori ved bestemmelse av kontrastmengde på CT-abdomen?

Formål: Drøfte kvaliteten av to protokoller for dosering av jodholdig kontrast ved CT-abdomen.

Metode: Kvalitativ litteraturstudie med analyse av vitenskapelige artikler.

Resultat: Kontrastoppladning i lever under portovenøs fase har en sterk sammenheng med kroppsindeksene TBW (r = - 0.69 +/- 0.05), LBW (r = - 0.68 +/- 0.09) og BSA (r = - 0.73 +/- 0.09). Bruk av en protokoll med fast-dose fører til interindividuell variasjon.

Konklusjon: Kroppskomposisjons-protokoll kan føre til redusert interindividuell variasjon i kontrastoppladning blant pasienter og lavere forbruk av kontrastmiddel.

Topic (research question): When determining amount of iodinated contrast media, what are the advantages of using a body-composition tailored dosing protocol instead of a weight-based dosing protocol?

Purpose: To investigate the quality of two protocols for calculating the dose of iodinated contrast media for patients undergoing an abdominal CT-scan.

Method: Qualitative literature study with analysis of scientific articles.

Results: During the portal venous phase, a strong correlation was exhibited between mean hepatic enhancement and the body indexes TBW (r = - 0.69 +/- 0.05), LBW (r = - 0.68 +/- 0.09) and BSA (r = - 0.73 +/- 0.09). The use of a fixed-dose-protocol leads to interpatient variability.

Conclusion: A body-composition tailored protocol may reduce interpatient variability in terms of mean hepatic enhancement and lead to less consumption of contrast media.
