Tittel: Hvordan miljøterapeutiske tiltak kan redusere risikoen for vold og aggresjon hos innlagte pasienter ved psykiatriske sykehus.

Introduksjon/bakgrunn: Vold blant pasienter ved psykiatriske sykehusavdelinger er et omfattende og utfordrende fagområde. Det å utøve adekvat behandling for pasienter med risiko for vold og aggresjon er spesielt vanskelig da det er nødvendig å ha tett oppfølgning for pasienter med psykiske lidelser. Mange ulike tiltak benyttes for å redusere risikoen for utagering i psykiatrien i form av atferdsterapi, kognitiv terapi, restriktive tiltak og miljøterapi.

Hensikt: Hensikten med denne bacheloroppgaven er å vurdere effekten av miljøterapeutiske tiltak for å redusere risikoen for vold og aggresjon hos pasienter innlagt i psykiatrien.

Metode: I denne oppgaven ble det benyttet en litteraturstudie for å finne fagfellevurderte artikler som belyste problemstillingen. Systematiske søk ble benyttet for å finne fem forskningsartikler av nyere opprinnelse. Fire kvalitative artikler og en kvantitativ artikkel ble benyttet i dette studiet.

Resultater: Funnene i de ulike artiklene gir indikasjoner på at miljøterapeutiske tiltak kan være hensiktsmessige tiltak for å redusere risikoen for vold og aggresjon hos pasienter som har risiko for å utøve vold.

Konklusjon: Miljøterapeutiske tiltak kan tilsynelatende minske risikoen for vold i psykiatrien. Kommunikasjon, relasjoner, holdninger, oppdage tidlige varselsignaler, samhandling og det psykososiale miljøet er faktorer som blir ansett som tiltak for å redusere risikoen for vold og aggresjon. Hvorvidt miljøterapien er den primære årsaken til reduksjonen for voldelig atferd, eller om det skyldes annen behandling, er derimot utfordrende å konkludere med, da pasientene i undersøkelsen mottok både medikamentell og andre former for terapeutisk behandling samtidig. Dette og et noe snevert antall deltakere i enkelte av studiene gjør at man derfor ikke kan slå fast den generaliserende effekten av miljøterapi mot vold og aggresjon.

Abstract

Title: How a therapeutic milieu can contribute to reducing violence and aggression in interaction with patients in psychiatric hospital wards.

Introduction/background: violence amongst patients in psychiatric wards is a comprehensive and challenging part of psychiatric care. It is particularly difficult to preform adequate treatment for patients with a high risk of committing violence and aggression because patients with mental illnesses needs close monitoring from the nursing staff. Many different actions are implemented to reduce the risk of violence and aggression such as behavioral therapy, cognitive therapy, restrictive actions and therapeutic milieu.

Purpose: the purpose with this essay is to measure the effects of how a therapeutic milieu can reduce the risk of violence and aggression in interaction with patients in psychiatric wards.

Method: In this assignment a literature study was conducted to find science-based articles that enlightened the main issue. Systematic searches were conducted to find five articles of new origin. Four qualitative articles and one quantitative were used in this study.

Results: The findings in the articles suggest that therapeutic milieu may have a positive effect in reducing the risk of violence and aggression in working with patients who have a high risk of committing violence.

Conclusion: a therapeutic milieu may apparently reduce the risk of violence in psychiatric treatment. Communication, relationships, attitudes, early warning signs, interaction and the psychosocial environment are factors that is considered important towards reducing the risk of violence and aggression. It is difficult to conclude if the therapeutic milieu is the main course of reducing violent acts because the patients in the surveys receive both medical and other forms of therapeutic treatment. This combined with a small number of participants in some of the studies, suggest that it may be difficult to generalize the effect of a therapeutic milieu in reducing violence and aggression.
