Den nye hverdagen - Livet med stomi

Abstract

Innledning: Det å få stomi er et tabubelagt tema. Det kan være nedverdigende å miste kontrollen over avføringen, og dette kan være med å påvirke selvbildet. Hensikt: Oppgaven er en litteraturstudie. Hensikten er å fremheve hvordan en sykepleier kan påvirke selvbildet til en pasient med ileostomi, og om informasjon er viktig. Metode: Det er søkt i databasene Cinahl Complete, SveMed+, PubMed og Academic Search Complete med søkeordene nurse, information, informasjon, ostomy, surgery, operasjon, permanent ostomy, ostomy surgery, body image, ileostomy, young adults, inflammatory bowel disease og education. Resultat: Pasientene opplevde å få for lite og mangelfull informasjon postoperativt. Flere uttrykte sjokk i det første møte med stomien. Det viste seg at noen følte seg mindre attraktive på grunn av arrdannelse og ukontrollert tarmfunksjon. Flere av pasientene var misfornøyd med at sykepleierne ikke så ut til å bry seg, og de opptrådte uprofesjonelt. Pårørende var gode støttespillere og de inspirerte pasientene til å akseptere den nye hverdagen. Konklusjon: Det ble flere ganger omtalt at informasjon er viktig, men dette forutsetter at informasjonen er relevant og tilpasset den enkelte. Noe som påvirket selvbildet til pasientene var de kroppslige forandringene og sjokket de fikk etter stomioperasjonen. Nærmeste pårørende har en stor påvirkningskraft, og sykepleieren bør bruke de som en ressurs postoperativt. Det som påvirket selvbildet positivt var at sykepleieren var tilgjengelig for å snakke om situasjonen, og at hun kom med oppløftende ord og positive tanker om fremtiden.

Introduction: Having an ostomy is taboo. It can be degrading to lose the ability to control their bowel function, and this can affect the self-image. Aim: This thesis is a literature review. The aim is to show how a nurse can affect the self-image of a patient with ileostomy, and if information is important. Method: To find relevant articles these databases were used: Cinahl Complete, SveMed+, PubMed and Academic Search Complete. Keywords were nurse, information, informasjon, ostomy, surgery, operasjon, permanent ostomy, ostomy surgery, body image, ileostomy, young adults, inflammatory bowel disease and education. Results: Patients received inadequate information postoperative. Several were in shock after their first meeting with ileostomy. Some felt less attractive because of the scarring and the loss of control over their bowel function. Several patients were dissatisfied with the nurses because they seemed not to care about their patients, and behaved unprofessionally. Next of kin gave support and inspired the patients to accept their new life. Conclusion: The importance of information was mentioned several times, but it has to be relevant and adjusted to each individual. Something that can affect the patient’s self-image is the change of body appearance and the shock after the ostomy surgery. Next of kin have a huge influence on the patients, and the nurses should use the next of kin postoperative. The nurse had a positive affect on the patients self-image with their availability to talk about the situation, and their positive words and thoughts about the future.
