Hvordan kan sykepleier bidra til at pasienter, i forbindelse med stomioperasjoner, kan mestre stomistell og akseptere sin stomi?

Abstract

Innledning: Det antas at om lag 10 000 nordmenn lever med stomi. Å få stomi er forbundet med mye usikkerhet, og de fleste vil oppleve det som skremmende. En stomioperasjon representerer tap av funksjon, og pasienten må derfor med tiden akseptere stomien og mestre stomistell. Hensikt: Belyse hvordan sykepleier kan hjelpe pasienter å mestre stomistell og bidra til aksept av stomi, i forbindelse med stomioperasjoner. Metode: Oppgaven er et litteraturstudie. Det er utført søk i databasene Ovid Medline, Cinahl Complete, PubMed og SveMed+ med søkeord som stoma/ostomy, stoma care, acceptance, education og nurs*. Resultater: Stomistell kan oppleves som ukomfortabelt i den postoperative perioden, og pasienten kan føle seg overveldet, frustrert og hjelpeløs. Mye informasjon postoperativt kan bidra til at pasienten føler seg overveldet, og dette kan forebygges ved hjelp av adekvat preoperativ informasjon og veiledning. Det har vist seg at enkelte pasienter mottok lite hjelp for å bli selvstendige i stomistell, og dette fremstår som viktig fordi mestring av stomistell er relatert til stomiaksept. Bruk av humor, anerkjennelse av positive endringer og fokus på normalisering kan fremme stomiaksept. Konklusjon: Stomiaksept er svært individuelt, og sykepleien som gis bør være individualisert og relatert til pasientens behov. Mestring av stomistell kan bidra til stomiaksept, og sykepleier bør rette fokus mot å hjelpe pasienten til å mestre stomistell. Det er funnet mye forskning angående stomi og psykososiale konsekvenser, men derimot lite om fremgangsmåter og sykepleietiltak for å hjelpe pasienten til å akseptere sin stomi. Det antas derfor at det er et behov for flere kvalitative studier som kan bidra til ytterligere kunnskapsutvikling angående stomiaksept.

Introduction: It is believed that about 10 000 Norwegians are living with a stoma. Receiving a stoma is associated with uncertainty, and most people will experience it as frightening. A stoma surgery represents loss of function, and the patient must eventually accept the stoma and learn to handle the stoma. Purpose: The aim of this literature review is to illuminate how nurses can help patients to master stoma care and contribute to stoma acceptance, in connection with stoma surgery. Method: This thesis is a literature review. Literature search is carried out in Ovid Medline, Cinahl Complete, PubMed and SveMed+. Keywords were stoma/ostomy, stoma care, acceptance, education and nurs*. Results: Stoma care can be uncomfortable in the postoperative period, and the patient may feel overwhelmed, frustrated and helpless. Too much information given in the postoperative fase can contribute to the patients feeling of being overwhelmed, and this can be prevented by adequate preoperative information and guidance. Results show that some patients received little help to become independent in stoma care, and this appears to be important because mastering of stoma care is related to stoma acceptance. Use of humor, appreciation of positive changes and focus on normalization can contribute to stoma acceptance. Conclusion: Stoma acceptance is individual, and the provided nursing should be individualized and related to the patient´s needs. Coping with stoma care contributes to stoma acceptance, and the nurse should focus on helping the patient to cope with stoma care. It is found a lot of research regarding stoma and psychosocial consequences, but less about procedures and nursing measures to help the patient accept their stoma. It is assumed that there is a need for more qualitative studies that can contribute to further knowledge regarding stoma acceptance.
