Liverpool Care Pathway: et nyttig verktøy ved livets slutt?

Abstract

NORSK: Hensikt: Vi er tre sykepleiere som jobber i sykehjem og sykehus. I vår arbeidshverdag har vi savnet et verktøy som kan hjelpe oss å strukturere omsorg for døende. I utdanningen ble vi kjent med Liverpool Care Pathway som er en tiltaksplan i omsorg for døende. Vi ønsket å undersøke helsearbeideres erfaringer ved bruk av LCP for å se om dette verktøyet kunne vært nyttig å innføre ved våre arbeidsplasser. Metode: Dette er en litteraturstudie med oppbygging etter IMRaD-prinsippet. Vi har valgt fem forskningsartikler ut fra kriterier om relevans og holdbarhet. Benyttede søkeord er: «Liverpool Care Pathway», Nurs*, experience, effect, benefits, advantage og disadvantage. Egenvalgt litteratur er også benyttet. Funn: Funnene i artiklene viser at symptomlindring, dokumentasjon og kommunikasjon med pasient, pårørende og med tverrfaglig team ble forbedret som følge av implementering av LCP. Å avgjøre når en pasient er døende ble i en artikkel imidlertid påpekt som problematisk. Konklusjon: Ut fra funnene i artiklene og egne tanker, ser vi på LCP som et godt rammeverk som kan være til hjelp for å sette omsorg for døende i et system som sikrer den terminale pasient kvalitet i omsorgen, spesielt i forhold til symptomlindring. Samtidig ser vi noen begrensninger og behov for mer forskning.

ENGLISH: Aim: We are three nurses who are working in nursing home and hospital. In our daily work, we are missing a tool to structure the care of the dying. In our education in palliative care, we learned about the Liverpool Care Pathway. This is an integrated care pathway in care of the dying. We wanted to examine health workers experience of using LCP, and to see if this tool could be useful to introduce at our places of work. Methods: This is a literature review with the use of the IMRaD structure. We selected five research articles out of criteria of relevance and durability. Search terms are:” Liverpool Care Pathway”, nurs*, experience, effect, benefits, advantage and disadvantage. Self-selected literature are also used. Findings: The results of the chosen articles showed that symptom control, documentation and communication with patient, relatives and with the multidisciplinary team was improved after implementation of the LCP. To find the right time of diagnosing dying, was seeing as a problem in one article. Conclusion: Based on the findings of the articles and our own thoughts, we look at the LCP as a nice framework that may help us to put the care of dying in a system that ensures the terminal patient quality of care, especially in terms of symptom relief. We also see some limitations and the need for more research.
