Sykepleiesammenfatning - et nyttig verktøy?: sykepleiers oppfatning om kommunikasjon mellom sykehus og sykehjem

Abstract

NORSK: Bakgrunn: Samhandlingsreformen i Norge medfører kortere liggetid på sykehus og en økt utskriving/overføring av pasienter med sammensatte behov. Dette krever at informasjonen som følger pasienten, er nøyaktig og lett å forstå for helsepersonell uavhengig av tid og rom, slik at kontinuitet i pleien kan opprettholdes. Hensikt: Hensikten med vår studie var å se på hvordan sykepleiere oppfatter kommunikasjon mellom sykehus og sykehjem, og om sykepleiesammenfatningen er et godt nok redskap for sykepleiere i sykehjem til at de føler seg trygg på at pasientopplysningene som er dokumentert speiler pasientens problemer og ressurser. Metode: Det er gjort en tverrsnittstudie blant sykepleiere ansatt i sykehjem i et fylke i Norge. Et spørreskjema fra USA er oversatt til norsk og brukt i en spørreundersøkelse mai til oktober 2012. Dataanalysen er framstilt med deskriptiv analyse. Svarprosenten var på 48 (n=82). Resultat: 80 prosent av de som svarte var i alderen 30 – 59 år, og 51 prosent av de som svarte hadde over ti års arbeidserfaring som sykepleier. De yngste (under 40 år), syntes det var vanskeligere enn de eldre å identifisere pasientens nåværende diagnose i sykepleie-dokumentasjonen. De med høyest alder anser sykepleiedokumentasjon for å være et bedre arbeidsredskap enn de med lavere alder. 60 prosent av respondentene ville at det alltid skulle være verbal kommunikasjon i tillegg til den skriftlige. Sykepleiedokumentasjonen blir generelt sett på som viktig når det gjelder planlegging av pleien av pasientene. 38 prosent var usikre på om viktig informasjon går tapt ved overføring fra sykehus til sykehjem. Konklusjon: Sykepleiesammenfatningen blir oppfattet som et godt arbeidsredskap og viktig for hvordan vi planlegger pleien av pasientene, til tross for at en høy andel ønsker verbal kommunikasjon i tillegg til den skriftlige, og at mange er usikre på om viktige opplysninger går tapt i en overføring.

ENGELSK: Background: The Coordination Reform in Norway brings about fewer bed-days in hospital and increased transfer of patients with complex needs. This requires that the information that follows the patient is accurate and easily understandable for health professionals independently of time and space, so that continuity in the care can be sustained. Aim: The intention with our study was to look into how nurses interpret communication between hospital and nursing home, and if the nursing documentation is an adequate tool for nurses in nursing homes, so that they feel confident that the documented information reflects the patient’s problems and resources. Method: A cross-sectional survey among nurses employed in nursing homes in Oppland county has been carried out from May to October 2012, using a US questionnaire translated into Norwegian. The data analysis is produced with descriptive analysis. The response rate was 48 (n=82). Result: 80 per cent of the respondents were 30–59 years of age, and 51 per cent of the respondents had over ten years of work experience as nurses. The youngest (under 40 years of age) found it more difficult than the older to identify the patient’s current diagnosis in the nursing summary. The oldest consider the nursing summary to be a better tool than the younger. 60 per cent of the respondents wanted verbal communication in addition to the written on regular basis. The nursing documentation is generally regarded as important with regards to planning the care of the patients is regarded. 38 per cent were unsure whether important information is lost at transfer from hospital to nursing home. Conclusion: Despite the fact that a high percentage wants verbal communication in addition to written communication, and that many are unsure whether important information is lost in a transfer, the nursing documentation is regarded as a suitable tool and important for how we plan the care of the patient.
