Sammendrag
NORSK: Overordnet hensikt er å bidra til at eldre sykehuspasienter med sammensatt
sykdomsbilde får helsehjelp som står i forhold til sammensattheten. Målsetting er å
integrere rehabiliteringstenking omkring pasienten. Masteroppgaven utforsker
hvorvidt dette lar gjøre etter helsefremmende prinsipper i en diagnostisk kultur. Er
det mulig å bevare og utvikle kunnskap som valideres gjennom tverrfaglig
kommunikasjon og samhandling, og som har betydning for klinisk praksis og
organisering av denne? En diagnostisk kultur kan sies å ha et naturvitenskapelig
kunnskapssyn; et diagnoseperspektiv på helseproblemer, markedstenkning ved at
diagnoser også er en økonomisk kategori, og ”Kunnskapsbasert praksis som
metode”, med foretrukne forskningsdesign og kriterier for validering av kunnskap.
Metode: Feltarbeid i egen kultur knyttet an til aksjonsforskning.
Resultat: I praksisfeltet valideres kunnskap også gjennom kommunikasjon. Det ser
ut til at kommunikativ validitet tematiseres og begrepsfestes lite. I formidling
fagpersoner i mellom gjerne med en ”språkblanding” av bedriftsøkonomiske og
helsefaglige termer.
Konklusjon: Det lar seg gjøre å fastholde/bibeholde et perspektiv komplementært til
den diagnostiske kultur gjennom systematisk kommunikasjon og samhandling satt i
system over tid. ENGELSK: The main intention of this work is that elderly people who are hospitalized with
complex diagnoses, receive health care proportional to the complexity of their
diagnosis. The aim is to integrate plans for rehabilitation into the patient’s treatment.
This work explores if this is possible when following Health Promotional principles
in a diagnostic culture. Is it possible to preserve and develop knowledge validated
through multidisciplinary communication and cooperation? Is this knowledge of
importance for clinical practice and the organizing of it? The diagnostic culture can
be said to be dominated by natural science approaches; a perspective for diagnosing
health problems, market oriented thinking by making diagnoses an economic
category, and «evidence based practice as a method», with preferred scientific design
and criteria for validation of knowledge.
Method: Fieldwork in own culture based upon action research.
Result: In the field of practice, knowledge is also validated through communication.
It seems that communicative validity is little thematised and is not tied to any
specific terms. Interaction between professionals is a «mix of languages», using both
economic terms and terms from the health profession.
Conclusion: It is possible to keep a complementary perspective towards the
diagnostic culture through systematic communication and cooperation set in a
system over time.
