Håndhygiene i hjemmebaserte tjenester

Abstract

NORSK: Forskning viser at etterlevelsen av riktig håndhygiene blant helsepersonell er lav. Norsk helsevesen er i endring, noe som medfører mere pasientbehandling i hjemmet, med større fare for infeksjoner påført av helsepersonellet. Håndhygiene er det mest effektive tiltaket for å hindre slike infeksjoner. De hjemmebaserte tjenestene ser ut til å oppleve hindringer i forhold til å utføre håndhygiene, men det finnes lite forskning på området. Hensikten er å finne ut om hjemmebaserte tjenester etterlever bruk av riktig håndhygiene, hvilke faktorer som gjør etterlevelse vanskelig, og om praktiske tiltak som opplæring, veiledning og tilrettelegging øker etterlevelsen. Metoden er et litteratursøk, med søkeord som primary healthcare, infection, hand hygiene, home based nursing care, home based services, compliance, guidance, education, healthcare-associated infection, intervention og community nurses. De fremste hindringene for å utføre håndhygiene i hjemmebaserte tjenester er mangel på oppdatert kunnskap, dårlig tilrettelegging og høy arbeidsbelastning. Praktiske tiltak bedrer etterlevelsen av riktig håndhygiene.

ENGELSK: Research shows that compliance with proper hand hygiene among health care workers is low. The Norwegian health care system is changing, resulting in increased home-care, with a greater risk of infections caused by the healthcare staff. To prevent these infections, correct hand hygiene is believed to be most efficient. The purpose of this paper, is to determine if hand hygiene compliance is applied correctly, the factors that make compliance difficult, and if practical measures such as training, mentoring and proper arrangement help to increase compliance. The method is a literature search, with keywords such as primary health care, infection, hand hygiene, home based nursing care, home based services, compliance, guidance, education, healthcareassociated infection, intervention and community nurses. The main difficulties of applying hand hygiene in the home-based services is the lack of knowledge, poor arrangements and a high workload. Practical measures will improve the compliance of a correct hand hygiene.
