Hvordan kan sykepleier forebygge fotsår hos pasienter med diabetes type-2?

Abstract

NORSK: I denne oppgaven har fokuset vert hvordan sykepleier kan forebygge fotsår hos pasienter med diabetes type-2. Jeg redegjør for undervisning og veiledning som er en viktig del av forebyggingen. Forklarer senkomplikasjonenen nevropati og aterosklerose og hvilke symptomer disse har. Jeg beskriver undersøkelser og utfordringer som en sykepleier har i forbindelse med identifisering av fotsår på pasienter med diabetes type-2. Jeg har brukt artikler fra Tidskrifter for den Norske Legeforening der fagpersoner på dette feltet har sterke meninger. Har valgt Virginia Henderson som sykepleieteoretiker, da hennes teori inneholder de grunnleggende behov, kunnskaper, krefter og vilje for å opprettholde en god livskvalitet et menneske trenger. Jeg har hospitert på en diabetespoliklinikk for å få innblikk i hvordan en sykepleier jobber med forebygging av fotsår som kommer av senkomplikasjonene nevropati og aterosklerose. Ut i fra dette ville jeg fordype meg i teorien for å tilegne meg kunnskap om hvordan jeg som sykepleier kan utføre denne forebyggingen på best mulig måte. Hovedkonklusjonen er at forebyggingen og behandling starter når diagnosen er stilt, for å redusere utviklingen av fotsår og eventuelt amputasjon. Forutsetningen for at dette skal lykkes ligger i kompetansen til alle involverte parter.

ENGELSK: In this subject my focus has been on how nurses can prevent foot wounds on patients with diabetes type-2. I will give an account for the education and guidance, which are important parts of the prevention. I will describe late complications as neuropathies and arterioscleroses and which kind of symptoms they have. I will describe the examinations and challenges a nurse has when it comes to the identification of foot wounds on patients with diabetes type-2. I have used articles from magazines from “den Norske Legeforeningen” (The Norwegian Medical Association) where specialists on this field have strong opinions. I have chosen Virginia Henderson as a nursing theorist because her theory contains the basic need, knowledge, power and will to maintain the good quality of life that a man needs. I have done my practise on a polyclinic for diabetes to get an impression of how a nurse works to prevent a foot wound that is caused by the late complications neuropathies and arterioscleroses. Based on this, I will go into the theory to obtain knowledge on how I as a nurse can execute this prevention in the best way possible. The main conclusion is that prevention and healing starts when the diagnostic is done in order to reduce the development of foot wounds and an eventual amputation. The condition of success is based on the qualifications of all parts involved.
