dc.description.abstractSammendrag Siden midten av 1980-tallet har Helseundersøkelsen i Trøndelag (HUNT) samlet inn helsedata fra frivillige nordtrøndere for å kunne forske på et bredt spekter av helserelaterte tema. Dette er et betydningsfullt forskningsprosjekt som omfatter en stor samling data fra et stort antall mennesker. Målet med studien i denne doktorgradsavhandling har vært å undersøke hvordan temaet risiko for sykdom kommer til uttrykk i ulike typer skriftlig og muntlig materiale. Studien som presenteres i doktorgradsavhandlingen er gjort innenfor den faglige disiplinen anvendt språkvitenskap. Materialet er hentet fra den tredje helseundersøkelsen i Nord-Trøndelag (HUNT3). Som avhandlingens tittel peker på representerer materialet en kommunikasjonskjede, med ulike skriftlige tekster til befolkningen både før, under og etter deltakelse, og transkriberte lydopptak av legekonsultasjoner med deltakere som har kontaktet fastlegen sin for individuell oppfølging av helseresultatene. Analysene av de skriftlige HUNT-tekstene viser hvordan HUNT rekrutterer deltakere, informerer om HUNT3 til befolkningen, og hvordan de gir tilbakemelding om resultatene fra den individuelle helseundersøkelsen til de som deltok. Et mønster i materialet er tekstenes retoriske karakter, hvor både ordlyd og bruk av bilder bidrar til det kommunikative prosjektet å overbevise om å delta i helseundersøkelsen. Analysene viser at HUNT løfter frem nytten av nordtrøndernes deltakelse, for eksempel gjennom en diskursiv innramming av HUNT3 som en felles dugnad, mens individuelle aspekter ved å delta slik som oppdagelse av sykdom, risiko for sykdom og ulemper ved å delta berøres i mindre grad. I brevet som deltakerne mottar med tilbakemelding om de individuelle resultatene av helseundersøkelsen, formidles prøveresultatene i form av tall og suppleres med generell informasjon om hver prøve. Deltakerne anmodes om å selv ta ansvar for resultatene, både gjennom en eventuell anbefaling om å kontakte fastlegen dersom prøveresultatene tilsier det, og gjennom generelle råd om hvordan man kan ta vare på helsen. Brevet peker mot fastlegekonsultasjonen som et sentralt ledd i oppfølgingen av den enkelte HUNT-deltaker. Tallverdiene kan medføre usikkerhet og spørsmål hos deltakerne, og fordi man i HUNT rekrutterer bredt, betyr det også at mange som i utgangspunktet anser seg selv for å være friske, blir klar over at man er i risikosonen for en sykdom. Analysen av fastlegekonsultasjonene gir innsikt i hvordan pasienter og lege orienterer seg mot sykdomsrisiko som tema, og hvordan risiko forstås og forhandles om. Dette kommer spesielt frem når HUNT-resultatene vurderes og forklares, og gjennom pasientenes uttrykk for uro og bekymring for resultatene. Analysen viser at forklaringer og vurderinger av pasientens sykdomsrisiko er en sentral aktivitet i legekonsultasjonene, sammen med håndtering av bekymring og uro. Legen tar i bruk ulike diskursive ressurser i disse forklaringene, for eksempel normalisering og omformulering av resultatene, og forsterkende og dempende ytringer. Et annet sentralt trekk i konsultasjonene er hvordan legen fritar pasientene for det moralske ansvaret for HUNT-resultatet, og hvordan dette ser ut til å være en diskursiv ressurs med hensyn til å berolige pasientene. Til sammen viser analysen av konsultasjonene at samtalen med fastlegen er et viktig ledd i HUNTs kommunikasjonskjede med hensyn til den reelle vurderingen, og deltakernes egen forståelse, av sykdomsrisiko. Som helhet gir studien innsikt i hvordan de kommunikative leddende i HUNT3 håndterer kommunikative utfordringer i tilknytning til temaet sykdomsrisiko. Det empiriske grunnlaget består av både tekster og samtaler, og de analytiske kategoriene er hentet fra både tekst- og samtaleforskning. Analysen av rekrutteringsmateriell, nettsider og brevet med tilbakemelding bidrar til forståelsen av skrevne teksters status i en kommunikativ virksomhet, og hvordan disse utgjør en sosiokulturell kontekst for legekonsultasjonene. Avhandlingen er også et bidrag til forskningen på kommunikative virksomheters multimodale kjennetegn, så som samspillet mellom dokumenter, bilder, skjema og samtaler. Overordnet gir studien kunnskap om kommunikative problemstillinger knyttet til store helseundersøkelser av en befolkning, og kan gi grunnlag for evaluering av gjeldende praksis. Summary Since the mid-1980s, the Trøndelag Health study (HUNT) has collected health data from a large number of volunteers from North-Trøndelag, which has been of high value for research within a wide range of health-related topics. The aim of the study in this doctoral thesis was to examine how risk of disease is expressed in various types of written and oral material from the HUNT study. The study presented in this thesis is within the professional discipline of applied linguistics. The material was gathered from the third health study in North-Trøndelag (HUNT3). As the thesis title indicates, the material represents a chain of communication, including written texts given to the population before, during and after participation, and transcribed audio recordings from medical consultations between participants and their general practitioner (GP) during individual follow-up of the health survey results. The analyses of the written texts describe how HUNT recruit and inform participants, and how they provide feedback about the individual health examination to those who participated. One interesting pattern found in the material is the rhetorical character of the texts, where both the wording and use of images contribute to the communicative project of convincing individuals to participate in the health examination. The analyses show how HUNT highlights the benefits of participation, for example through a discursive framing of HUNT3 as a joint effort, while individual aspects of participating such as discovery of disease, risk of disease and disadvantages of participating are touched upon to a lesser extent. In the letter that the participants receive, which contains feedback on their individual results of the health examination, test results are communicated in the form of numbers and supplemented with general information about each test. The participants are expected to take responsibility and act on the results themselves; this is indicated in the recommendation to participants to contact the GP regarding test results, and through the general advice provided on how participants can care of their own health. The letter points to the GP consultation as a central link in the follow-up of the individual HUNT participant. The communication of numerical values can lead to uncertainty and questions among the participants, and because HUNT recruits widely, it also means that many people who initially consider themselves to be healthy become aware that they are in the risk zone for an illness. The analysis of GP consultations provides insight into how patients and doctors orientate themselves towards disease risk as a theme, and how the notion of risk is understood and negotiated. This is particularly apparent when the HUNT results are assessed and explained by the GP, and through the patients' expression of worry and concern about the results. The analysis shows that explanations and assessments of the patient's risk of disease is a central activity in the doctor's consultations, in addition to dealing with patient worry. The doctor makes use of various discursive resources in these explanations, for example normalization and reformulation of the results, and intensifying and mitigating expressions. Another central feature of the consultations is how the doctor relieves the patients of the moral responsibility for the HUNT result, and how this appears to be a discursive resource for reassuring the patients. Altogether, the analysis of the consultations shows that the conversation with the GP is an important link in HUNT's chain of communication with regard to assessment and the participants' own understanding of disease risk. The study provides insight into how the communicative links in HUNT3 handle challenges connected to communication of risk of disease. The empirical basis consists of both texts and conversations, and the analytical categories are taken from both text and conversation research. The analysis of recruitment material, websites and the participant feedback letter contribute to the understanding of the status of written texts in a communicative practice, and how these constitute a socio-cultural context for the medical consultations. The thesis is also a contribution to research into the multimodal characteristics of communicative practices, such as the interaction between documents, images, forms, and conversations. Overall, the study provides knowledge about communicative issues and challenges related to population health surveys and can provide a basis for evaluating current practice.en_US
