Sykepleiers ivaretakelse av pårørende til palliative pasienter

Abstract

Introduksjon: Det overordnede målet innenfor palliasjon er best mulig livskvalitet for pasienter og pårørende i møte med livstruende sykdommer. Hvordan sykepleiere ivaretar pårørende betyr mye for hvilken støtte de gir til sine nære, og for veien videre etter døden. Studien ser på hvordan sykepleier kan ivareta pårørende til palliative pasienter og hvilke betingelser som ligger til grunn for dette.

Metode: Et samarbeidsprosjekt mellom sykepleierutdanningen ved NTNU og et hospice. Det ble gjennomført fire semistrukturerte kvalitative intervjuer med sykepleiere som arbeidet ved hospicet. Ved bruk av systematisk tekstkondensering ble intervjuene analysert i hovedtema og undertema med tilhørende intervjutekst til hvert undertema.

Resultat: Resultatene viser at informantene prioriterer å ivareta pårørende ved hospicet på lik linje som pasienter. De tilbyr blant annet kost og losji for en pårørende, ulike samtaletilbud og generelt en tett oppfølging. De følger opp pårørende underveis og etter dødsfall. Kartleggelse og ivaretakelse av deres behov er vesentlig. Informantene ivaretar pårørende gjennom å inkludere de, god, tilpasset informasjon og å se og anerkjenne de som egne individer med egne behov. Sykepleiers egen relasjon til døden, tid og bemanning er faktorer som har betydning for utøvelsen av sykepleie hos pårørende til palliative pasienter.

Konklusjon: Det er ikke store avvik fra etablert teori. Funnene viser at hvordan informantene ivaretar pårørende samstemmer med faglitteraturen. Likevel viser annen forskningslitteratur at pårørende savner mer oppfølging fra helsepersonell, særlig når det kommer til informasjon. Videre forskning bør inkludere pårørendes perspektiv, flere institusjoner og et større utvalg.

Introduction: The overall goal in palliative care is to provide the best possible quality of life, to patients and their next of kin who are facing life-threatening illnesses. Nurses ability to care for the next of kin plays a role in how they support and care for their close ones. This study researches how nurses can take care of the next of kin of palliative patients, and which conditions creates the foundation for this.

Methodology: A collaborative project between the nursing program at NTNU and a hospice. There were conducted four qualitative semi-structured interviews with nurses who worked at the hospice. With the use of systematic text condensation the data was analyzed and categorized in main-themes and sub-themes with interview text belonging to each sub-theme.

Results: The results shows that the informants prioritized taking care of the next of kin equally as the patients. They offer support through different forms of accommodation and meals for one next of kin, a variety of support-groups, and generally a close follow up. They focus on covering their needs. They support the next of kin throughout the course, and follow up after death for a period of time. They focus on caring for them through good and adapted information, including them in the palliative care and by seeing and recognizing them as individuals with their own needs. Nurses own relation to death, time and staff were factors affecting palliative care.

Conclusion: The study has no major deviations from established theory. The findings show that how the informants care for the next of kin agrees with the established theory. However research-literature shows that next of kin wants more follow-up from health care personnel, especially when it comes to information. Further research should include the next of kin perspectives, other institutions and a broader selection.
