dc.description.abstract | Bakgrunn: Antimikrobiell resistens (AMR) er et av de største globale helseproblemene.
Uforsiktig bruk av antibiotika har ført til økt resistens blant mikroorganismene. Den økende
resistensen reduserer effekten av antibiotika som brukes og har implikasjoner i ulike
behandlingsformer. Verdens helseorganisasjon (WHO) har erklært AMR som en stor trussel
mot global helse og har annonsert en handlingsplan for å prøve å kontrollere problemet.
Pakistan står overfor en økende rate av AMR i landet som mange andre lav- og
mellominntektsland (LMICs). Selv om landet har tatt skritt i riktig retning ved å formulere en
nasjonal handlingsplan, er det mangel på implementering av politikken og begrensede
fasiliteter i helsesektoren som fører til en ukontrollert økning av AMR.
Metoder: Et kvalitativt studiedesign ble brukt for å utforske oppfatningene til helsepersonell
(leger), farmasøyter og pasienter i distriktet Gujrat i Pakistan. Totalt atten deltakere ble
intervjuet personlig. Intervjuene ble tatt opp med lyd, transkribert og deretter analysert ved
hjelp av tematisk analyse.
Resultater: Deltakerne delte sine synspunkter om AMR i samfunnet og potensielle
årsaksfaktorer. Ifølge dem begrensede fasiliteter i det offentlige helsesystemet, enkel tilgang
til reseptfrie antibiotika, vaner som selvmedisinering og dårlig samsvar, irrasjonell
forskrivning av antibiotika, tilstedeværelse av uautoriserte helsearbeidere, press på legene fra
pasienter og mangel på funksjonelle retningslinjer for AMR-kontroll er viktige faktorer som
er ansvarlige for uansvarlig antibiotikabruk og dermed AMR. De mente at disse problemene
må tas opp for å forbedre situasjonen i årene som kommer.
Konklusjoner: Denne studien fremhever determinantene for AMR og kunnskapen og
holdningene til de tre sentrale interessentene (helsepersonell, farmasøyter og pasienter) for
AMR i Gujrat. Mangel på kunnskap er en nøkkelfaktor som påvirker forbruksmønsteret av
antibiotika i befolkningen på en negativ måte. Derfor er det behov for å sette i gang
programmer for å øke bevisstheten rundt AMR og dens negative virkning. Styringen i
helsesektoren må forbedres for å gi bedre helsetjenester til befolkningen og for å sikre
utforming og implementering av politikk for å kontrollere uansvarlig bruk av antibiotika og
dermed AMR. | |
dc.description.abstract | Background: Antimicrobial resistance (AMR) is one of the major global health problems.
The non-judicious use of antibiotics has led to increased resistance among the
microorganisms. The growing resistance reduces the efficacy of antibiotics being used and
has implications in various treatment modalities. The World Health Organization (WHO) has
declared AMR as a major threat to global health and has announced an action plan to try to
control the problem. Pakistan, like many other low and middle income countries (LMICs), is
faced with an increasing rate of AMR in the country. Even though the country has taken steps
in the right direction by formulating a National action plan, there is a lack of implementation
of the policies and limited facilities in the health sector leading to an uncontrolled rise of
AMR.
Methods: A qualitative study design was used to explore the perceptions of medical
practitioners (doctors), pharmacists and patients in the district of Gujrat, Pakistan. A total of
eighteen participants were interviewed in person. The interviews were audio recorded,
transcribed and then analyzed using thematic analysis.
Findings: The participants shared their views on the presence of AMR in the community and
the potential causative factors. According to them, limited facilities in the public health
system, easy access to over the counter antibiotics, habits such as self-medication and poor
compliance, irrational prescription of antibiotics, presence of unauthorized health workers,
pressure on the doctors from the patients and lack of functional policies for AMR control are
important factors that are responsible for irresponsible antibiotic usage and thus AMR. They
felt that these issues need to be addressed to improve the situation in the coming years.
Conclusions: This study highlights the determinants of AMR and the knowledge and attitudes
of the three key stakeholders (medical practitioners, pharmacists and patients) for AMR in
Gujrat. The lack of knowledge is a key factor that influences the antibiotic consumption
pattern of the population in a negative manner. Thus, there is a need to initiate programs to
raise awareness around AMR and its negative impact. The governance in the health sector
needs to improve to provide improved health services to the population and to ensure
formulation and, more importantly, implementation of policies to control the irresponsible use
of antibiotics and hence AMR. | |
