Pasienter med stomi og utfordringer knyttet til seksualitet

Abstract

BAKGRUNN: Seksualitet blir sett på som en sentral del av menneskets identitet, og kan bli påvirket av både stress og sykdom. Å få en stomi vil påvirke livssituasjonen, selvbildet og forholdet til egen kropp. Problemer i seksuallivet kan være en belastning for den enkelte, men det kan også påvirke et eventuelt parforhold. For mange kan seksualitet være et intimt tema, og dermed vanskelig å snakke om. HENSIKT: Hensikten med denne bacheloroppgaven er å belyse utfordringer rundt seksualitet hos pasienter med stomi, samt øke sykepleiernes kunnskaper knyttet til temaet. METODE: Bacheloroppgaven er en litteraturstudie hvor det er benyttet både kvantitative og kvalitative forskningsartikler. Databasene Ovid Medline og Cinhal ble brukt i det strukturerte søket etter vitenskapelige artikler. For å belyse problemstillingen ble det inkludert fem artikler i litteraturstudien, samt ressurslitteratur, fagartikler og annen relevant litteratur. RESULTAT: Komplikasjoner med stomien kan føre til seksuell inaktivitet, endret selvbilde og nedsatt livskvalitet. I tillegg kan dette påvirke et eventuelt parforhold. Mange pasienter er mindre seksuelt aktive etter de har fått anlagt stomi på grunn av psyko-emosjonelle problemer. Samtidig er det et gap mellom pasientenes informasjonsbehov, og den informasjonen som sykepleieren gir. KONKLUSJON: Gjennom veiledningsverktøy som PLISSIT-modellen og BETTER-modellen kan sykepleiere åpne opp for samtale om seksualitet og identifisere pasienters behov for informasjon og veiledning. På den måten bidrar sykepleiere til at pasienter blir i stand til å ivareta sin seksualitet, og dermed får en bedre livskvalitet. Implementering av undervisning om temaet i sykepleierutdanningen kan bidra til å heve kompetansen, og kan på den måten senke terskelen for å ta opp temaet med pasientene.

BACKGROUND: Sexuality is seen as a central part of the human identity, and can be affected by both stress and illness. Getting a stoma will affect the life situation, self-image and relationship to one’s own body. Problems within the sexual life can be a burden on the individual, but it can also affect any relationship. For many, sexuality can be an intimate topic, and thus difficult to talk about. AIM: The aim of this bachelor’s thesis is to shed light on challenges related to sexuality in patients with stoma, as well as to increase the nurses’ knowledge related to the topic. METHOD: This bachelor's thesis is a literature study using both quantitative and qualitative research articles. The databases Ovid Medline and Cinhal were used in the structured search for scientific articles. To answer the research question, five articles were included in the thesis, as well as resource literature, academic articles and other relevant literature. RESULTS: Complications with the stoma can lead to sexual inactivity, altered self-esteem and impaired quality of life. In addition, this may affect a relationship. Many patients are less sexually active after having a stoma due to psycho-emotional problems. There is a gap between the patients’ information needs and the information provided by the nurse. CONCLUSION: Through guidance tools such as the PLISSIT model and the BETTER model, nurses can open up conversations about sexuality and identify the patients’ need for information and guidance. In this way, nurses contribute to patients being able to maintain their sexuality, and thus achieve a better quality of life. Implementing teaching on the topic of nursing education, can help raise competence and lowe the threshold for addressing this topic with patients.
