dc.description.abstractAbstrakt Bakgrunn: Et samarbeidsprosjekt mellom sykepleieutdanningen ved NTNU og et kommunalt helse og velferdssenter. Sykepleiedokumentasjon dokumenteres i et vidt spenn av strukturer og formasjoner. Tiltaksplanens struktur tar utgangspunkt i problemløsningsmetoden, sykepleieprosessen. Sykepleieprosessen innfrir kravene til dokumentasjon i henhold til pasientjournalforskriften. Hensikt: Målet var å få et innblikk i hvordan tiltaksplanen kan bidra til å bevisstgjøre sykepleierens bruk av sykepleieprosessen i den kliniske hverdagen. Metode: Studiens design er dybdeintervju med sykepleiere på en utvalgt avdeling på et kommunalt helse- og velferdssenter. Resultat: Studien viser at sykepleieinformantene synes tiltaksplanen er et godt arbeidsverktøy å dokumentere i samt at det bidrar godt til sykepleieutøvelsen deres. Tiltaksplanen oppdateres ikke kontinuerlig, noe som har blitt mer fokus på i avdelingen. Konklusjon: Studien indikerer at tiltaksplanen legger grunnlag for å være en god dokumentasjonsmetode, der sykepleieprosessen er aktivt tatt i bruk. Informantene er bevisst på deres sykepleieutøvelse, men ikke bevisst på at sykepleieprosessen er en del av deres utøvelse.
dc.description.abstractAbstract Introduction: A collaborative project between the nursing education at NTNU and the municipal health center. Nursing documentation is documented in many types of structures and formations. The structure of the intervention plan is based on the problem-solving method, the nursing process. The nursing process meets the requirements for documentation according to the patient record regulations. Objectives: The aim was to get insight into how the intervention plan can contribute to raising awareness of the nurse´s use of the nursing process, in their everyday clinical practice. Method: The design is an in-depth interview with nurses in a selected department at the municipal health center. Result: The study shows that the nursing informants think the implementation plan is a good documentation tool and that it contributes well to their nursing practice. The implementation plan is not continuously updated, which has become more in focus in the department at the municipal health center. Conclusion: The study indicates that the intervention plan lays a foundation for good documentation where the nursing process is actively used. The informants are aware that of their nursing practice, but not aware that the nursing process is a part of the practice.
