Anestesi-, operasjon og intensivsjukepleiarane sine erfaringar med kommununikasjonsverktøy ved overføring av pasientar frå operasjonsstua til intensiv/recovery

Abstract

Denne studien blei skrivet med ønske om å finne ut korleis rapporteringa går føre seg ved overføring av den pasienten frå operasjonsstua til intensiv/recovery. Samt korleis rapporteringa er gjennomført i lys av pasienttryggleiken. Studien er gjennomført i eit helseføretak på Vestlandet. Hensikta med denne studien er å utforske anestesi-, intensiv- og operasjonssjukepleiarane sine erfaringar med kommunikasjonsverktøy ved overføring av pasienten frå operasjon til intensiv/recovery. Forskingsspørsmålet er kva erfaring har anestesi-, intensiv- og operasjonssjukepleiarar med pasientoverføring frå operasjon til intensiv/recovery med bruk av ISBAR eller andre kommunikasjonsverktøy. Metode: Kvalitativ metode med semistrukturert fokusgruppeintervju med hermeneutisk tilnærming. Informantane i denne studien er seks anestesisjukepleiarar, sju intensivsjukepleiarar og fire operasjonssjukepleiarar med ulik ansiennitet og bakgrunn. Funna peikar på to områder som informantane skildra som særskilt viktig. Informantane ønsker at kommunikasjonsverktøyet ISBAR blir nytta saman med PISIC for å auke pasienttryggleiken. Dette med at pasientryggleiken blir forsterka når ein har ein strukturert mal å bruke når pasienten blir overført frå operasjon til intensiv/recovery. Det kom og fram at ufullstendig rapportering førte til ekstraarbeid for intensivpersonalet, som måtte bruke tid i ettertid på å finne ut av mangelfull informasjon. Konklusjonen er at det er ulikt korleis rapporteringa blir gjennomført. Kommunikasjonsverktøy som ISBAR vil bidra til en meir strukturert rapport. Dette kan auke pasienttryggleiken då pasienten som pasienten som blir overfør mellom ulike avdelingar er ekstra sårbare. Det er behov for å samanfatte rutinar for standardisert rapportering, både skriftleg, munnleg, samt det praktiske rundt det å gi rapport

This study was written due to a desire to find out how the report takes place with transfer of the newly operated patient from the operation ward to intensive/recovery unit. Also how the report is conducted in light of patient safety.The study took place at a health care trust on the west coast of Norway. Purpose: The purpose of this study is to explore anesthesia nurse, surgical nurse and intensive care nurse experience with communication tool when transfering the patient from the operating room to the intensive care unit. The research question is what experience does anesteshia nurse, operation nurse and intensive care nurse have with reporting of patient transfer from operation ward to intensive care unit/recovery. The method is a quality method is used with semi-structure focus group interviews wih a hermeneutic approach. The informants who participated were six anesteshia nurses, four surgical nurses and seven intensive care nurses with different seniority and experience. The results is that the finding pointed two main areas wich the informants pointed out as particulary important. The informants wanted ISBAR communication too to be combined with the electronis patient record, PICIS to increase patient safety. And second, that ISBAR can attribute that the oral, written and the practical way to give report is more equal. It was revealed thar inadequate communication led to extra work for the intensive care nurses, who had to spend time afterwards clarifying the missing information The conclusion is, it is different how the report is conducted. Communication tool as for example ISBAR will contribute to a more structured report. This can increase patient safety. Since patients wich are transferd between different levels are more vournerable There is need to summarize the routine for standarized reporting with both the written verbal transfer of communication, as well as the practicalities of giving a report.
