dc.description.abstractBakgrunn: Ytterligere validering av den diagnostiske ytelsen til det relativt nye Siemens ADVIA Centaur® High-Sensitivity Troponin I (TNIH) immunoassayet kan fremdeles gi nyttig informasjon før eventuell implementering i klinisk praksis på St. Olavs hospital HF. Vi hadde som mål å evaluere den diagnostiske nøyaktigheten til TNIH assayet for akutt hjerteinfarkt uten ST-elevasjon (NSTEMI = non-ST elevation myocardial infarction) hos pasienter med mistenkt akutt hjerteinfarkt basert på blodprøver tatt kort tid etter sykehusinnleggelse. Materiale og metode: Vi gjennomførte en pilotstudie i form av en prospektiv observasjonsstudie ved St. Olavs hospital HF, Universitetssykehuset i Trondheim, Norge. Pasienter i alderen 18 år eller eldre ble fortløpende registrert som aktuelle om de ankom sykehuset med mistenkt akutt hjerteinfarkt basert på rekvirering av en «infarktstatus»-analysepakke. Blodprøven for indekstesten ble tatt samtidig som standardblodprøver inkludert prøven brukt i gullstandarden. Skriftlig samtykke ble innhentet i ettertid. Gullstandarden var NSTEMI-diagnose uthentet fra epikrisen (diagnostikk utført av behandlende lege basert på kliniske funn og serieprøver av høysensitivt kardialt troponin T (hs-cTnT)). Datainnsamlingen ble gjort uten å vite utfallet til pasienten. Diagnostisk nøyaktighet ble evaluert ved å beregne arealet under Receiver Operating Characteristic-kurven (ROC AUC), samt sensitivitet, negativ prediktiv verdi (NPV), spesifisitet og positiv prediktiv verdi (PPV) ved relevante beslutningsgrenser. Resultater: Av 124 inkluderte pasienter var NSTEMI endelig diagnose hos 14 (11,3 %). Samlet AUC for TNIH kort tid etter sykehusinnleggelse var 0,93 (95 % CI 0,87–0,97). Ved å bruke 99-persentilen foreslått av TNIH assayets produsent fant vi en sensitivitet på 71,4 % (95 % CI 41,9–91,6), NPV på 96,0 % (95 % CI 91,4–98,2), spesifisitet på 88,2 % (95 % CI 80,6–93,6) og en PPV på 43,5 % (95 % CI 29,5–58,6). Diskusjon: Sammenliknet med liknende studier gjennomført på dette assayet viser denne pilotstudien en tendens til lavere samlet diagnostisk nøyaktighet vist ved ROC AUC for TNIH for NSTEMI. På grunn av den lille populasjonsstørrelsen, spesielt det lave antallet pasienter med NSTEMI, er punktestimatene for usikre til å trekke sikre konklusjoner. Før eventuell implementering i klinisk praksis på St. Olavs hospital HF er det derfor behov for større studier for å kunne bekrefte eller avkrefte disse tendensene.
dc.description.abstractBackground and Objectives: Further validation of the diagnostic performance of the relatively new Siemens ADVIA Centaur® High-Sensitivity Troponin I (TNIH) immunoassay might still yield useful information before possible implementation in clinical practice at St. Olav’s University Hospital. We aimed to evaluate the diagnostic accuracy of the TNIH assay for non-ST elevation myocardial infarction (NSTEMI) in patients with suspected acute myocardial infarction (AMI) based on blood samples collected shortly after hospital admission. Materials and Methods: We conducted a prospective, observational pilot study at St. Olav’s University Hospital in Trondheim, Norway. Consecutive patients aged 18 years or older presenting to the emergency department (ED) with suspected AMI based on the ordering of an “infarction status” analysis package was enrolled. The blood sample for the index test was collected at the same time as standard blood samples including the reference standard, retrospectively requesting written informed consent. A final diagnosis of NSTEMI extracted from the discharge summary (diagnostics performed by treating clinicians based on clinical findings and serial samples of high-sensitivity (hs)-cardiac troponin T) was used as the reference standard. Data were collected without knowing the patient outcome. Diagnostic accuracy was evaluated by calculating area under the Receiver Operating Characteristic curve (ROC AUC), and sensitivity, negative predictive value (NPV), specificity and positive predictive value (PPV) at relevant decision limits. Results: Of 124 included patients, NSTEMI was the final diagnosis in 14 (11.3 %). The overall ROC AUC for TNIH shortly after admission was 0.93 (95 % CI, 0.87–0.97). Using the 99th percentile suggested by the manufacturer of the TNIH assay, we found a sensitivity of 71.4 % (95 % CI, 41.9–91.6), NPV of 96.0 % (95 % CI, 91.4–98.2), specificity of 88.2 % (95 % CI, 80.6–93.6), and a PPV of 43.5 % (95 % CI, 29.5–58.6). Discussion: Compared to similar studies conducted on this specific assay this pilot study shows a tendency of lower overall diagnostic accuracy as quantified by the ROC AUC of the TNIH assay for NSTEMI. Because of the small population size, and especially the low number of patients with NSTEMI, the point estimates are too uncertain to draw definite conclusion. Larger studies are therefore needed to confirm or refute these tendencies before possible implementation in clinical practice at St. Olav’s University Hospital.
